First, I'd like to apologize if I'm posting this in the wrong place, if this is a little long-winded, or if I break etiquette. I'd like to pre-offer my sincerest apologies and (should I get any info I seek) my profound gratitude. Long time lurker, first time poster as I cannot seem to find information pertaining specifically to my problem.

I was recently diagnosed with mild narcolepsy. The most severe sleep attacks generally suck major shaft to trudge through but I can come out the other side still conscious and the minor ones are easily medicated with caffeine and a quick smoke though I still feel them. The reason I went through the majorly inconvenient and uncomfortable testing should be obvious: legal use of a scheduled drug, amphetamines. Legality is especially important due to my work administering random drug testing very often. I was first prescribed armodafinil (which made me groan inside) and handed me 2 weeks of samples, but luckily, through lack of pharmacy stock and awful monetary cost, I convinced my specialist to script me dextroamphetamine instead on the same day. To my delight, he started me on an insane initial dose of 4x10mg XR daily which at first seemed like an error of the pharmacy but in fact was entirely intentional (part of the reason I think my doc is completely pharm-ignorant). I could only get the Barr generic. That weekend I binged. I consumed probably about 80-100mg d-amp and 450-600 mg armodafinil in a 2 day period, mostly oral, snorted a few times. The armodafinil was probably a mistake, in retrospect, needless tolerance. Great time, empathy, euphoria, body high, etc. Horrible comedown as I didn't do my research and ended up severely dehydrated and half-starved, body pains like crazy. After this experience, began taking a ton of different supplements/vitamins though not L-Tyrosine cause I couldn't find it.

After discontinuing use for 3 days, I took 20mg to begin a 4 hour road trip and 20mg upon arrival for energy as the day would be tedious and long and I would need to drive back later. Effects noticeably diminished but still achieving small euphoria. Chain smoking like a nervous whore. Ended up crashing right before driving back (5 hours after last dose), so I took 30mg to make it back without falling asleep and dying, revived earlier effects. Made it home and crashed hard. Good water intake the entire time. Next day began taking magnesium 1000mg and DXM 30-60mg 2x daily for NMDA effects against tolerance.

Abstain for 3 days, taking 30mg+10mg at wake-up and around noon to make it through a day that promised to suck. Too long to feel un-sleepy effects (2 hours) and horribly short duration (<4 hours each dose). Very very mild euphoria and increased desire to talk. End up dying from sleep attacks at end of work day, take 40mg that night with a couple tablespoons of baking soda, a Nexium, and 2 tums to stay awake for company and card games. Mild euphoria, music amazing, horrible OCD behavior (cleaning), duration ~6 hours.

Abstain for another 2 days till today. Had a horrible sleep attack at about 1pm, took 20mg crushed beads dissolved in water. About half an hour later, I get very fuzzy headed, unable to focus, and extremely tired to the point where I had to stand up or I'd pass out. Barely made it home before I crashed hard and passed out for 2 hours. Awake now and still kinda shitty tired/achey feeling.

Sorry again for the story of my life. Could I have just bombed the hell out of my tolerance and need a longer reset time? Any chance my doc will script me memantine for anti-tolerance if he's pharm-retarded (I have RLS too, if that counts for anything)? Possible ADD/ADHD? Problem because it's generic or XR (come-up time/duration/minimal effects)? Something I'm doing wrong? Any sort of advice will help immensely. Thank you for your time.

I doubt that your doctor will give you alzheimer drugs, not sure if it's proven to reverse stimulant tolerance in narcoleptics or anyone.I think IR dextroamphetamine or IR methylphenidate(kind of weaker and shorter acting) is what is usually used for narcolepsy, up to 60mg a day.I think I read somewhere that stimulants don't have a tolerance effect against narcolepsy, though their aren't as many studies as with ADD(narcolepsy's pretty uncommon) and you did abuse it. If the narcolepsy isn't that bad you could take a break for a few days, a few day like a week break can drastically reduce tolerance.

There's also xyrem(sodium oxybate, phamacutical GHB), that's used at night, it's only used for narcolepsy.

My therapist and I have a suspicion that I'm ADD/ADHD because of the effects on my motivation/focus. I can only focus on things when it's got a constant feedback element and anything that doesn't causes me to feel increasingly like crawling out of my skin until I stop. It also helped with my social anxiety and depression, plus the effect is more of a calming alertness than anything. I feel like Adderall IR would be best for this with an NMDA antagonist and breaks either on weekends or for a week long period each month. But with my current docs, I'm not sure how to push this standpoint. My therapist who actually hears me out can't script meds, the GP is scared of anything to do with meds, especially powerful ones, and the sleep specialist is medication ignorant. I'm not too interested in abusing it if I could feel more of the drug other than just not feeling tired, which it doesn't even seem to work for anyway.
Could the problem be that it's XR?
I know about Xyrem, but I'm not really wanting to push for it as I'd have to exaggerate my nearly non-existant cataplexy, which is actually what it's scripted for. Not really interested in a sedative except maybe benzos for comedown and maybe sleep.

story of my life.. minus the narcolepsy + ADHD and minor depression. my binges can get quite out of hand, but yea its definitely turbo acute tolerance binges are hands down the fastest way to losing the magic. in your case it could lead to far worse consequences than mine

A few of the sentences in your post indicate you're taking stims for the euphoria more than for your narcolepsy. You've gone on a massive binge coupled with regular abuse, and now you're wondering why the stims aren't working anymore?

If you want to treat your narcolepsy (which I'm assuming is why you posted), you need to limit your prescribed medication to use specifically for narcolepsy. You also need a healthy body with plenty of energy, and this means eating well and no more poisoning yourself with DXM and the like in vain attempts to lower your stimulant tolerance. L-Tyrosine is found in meats and many other things, and supplements are not a substitute for eating well, so don't waste your money on them.

ADD/ADHD has nothing to do with the effect of stimulants, they affect all people more or less the same except for outliers.

If this narcolepsy is a serious problem (I feel for you man), you need to treat it seriously. That doesn't mean cleaning up your tolerance to amphetamine, it means cleaning up you, and progressing from there. Might I suggest extended-release stims, and exercising + eating fruit when you feel tired.

Or you could just keep doing what you're doing and suffer the extreme highs and lows.

Be careful with amphetamines! They're great when used effectively and responsibly, especially for the conditions you listed although you do seem to be abusing them like most people do when they first get prescribed something they're excited about.

Definitely important to keep up your healthy diet and exercise, amphetamines WILL take a toll on your body and especially your mind if you don't respect them. Try to have at least 2 amphetamine free days per week, that will work better for tolerance than any supplement you might take IME. You started out on dextro-amp, which is great because you didn't have to deal with the absolute HELL that is l-amp, which is why I think you should avoid ANY adderall product like the plague. If that modafinil-like drug you mentioned made you feel shitty, levo-amphetamine will make you feel shittier.
