Stimulants Concerta and Adderall

I was diagnosed ADD 13 years ago and I also have GAD/panic. I recently had a baby, so I've been med free for about a year now. I started Pristiq a couple months ago and so far really like it. I'm also on Klonopin. Now we are addressing my ADD. My doc put me on 27 mg Concerta and all I wanted to do was lie down. I was so sleepy. I went to see my doc today. He took me off the Concerta and back to Adderall IR. my question is, I took the 27 mg Concerta this morning and then the Adderall IR this afternoon. I know I shouldn't have mixed them. Is it super bad I mixed them (only 10 mg Adderall, I was just so tired from the Concerta).
 
It's not super bad just don't do it again. It's definitely not good but your dosages are low enough to where it shouldn't be too much.

Thank you Crashing. That's what I was hoping. I just can't wait for this Concerta to get out of my system. I feel so sleepy and hazy. I've been on Adderall in the past with much better results.
 
You should be fine, all that will happen is the methylphenidate (concerta) will negate the effects of the adderall.
 
I was diagnosed very late for ADD (obv because of the ADHD -H, so the signs weren't there when I was a kid).
Methylphenidate is my only option in my country.
I don't like the feeling at all and therefore am not taking medication for my ADD.
So you are not alone on this subject and if the Adderall works for you then use it sparingly.
I wouldn't worry too much about the Concerta dose, the worry would cause more "distress" than the physical symptoms at that dose.

Just don't take any more (Adderall) until it is time for your next Prescribed dose (if needed).
 
Not sure if you know about this yet, but Concerta is MPH based on extended-release mechanism, and I find mph medications with xr mechanism is pretty crap for most people. Complaints about consistency is the most common, which leads to pronounced side effects during the time-gap of the release mechanism. The side effects can be compounded into a severe "crash" at end of experience.

On the other hand, you should ask your doctor about MPH-sustained release medications. like Ritalin-SR. IMO, has all the positives of instant release tabs (hyper-focus, increased productivity), while minimizing the negatives (prolongs experience up to 3+ hours, diminishes the anxiogenic effects during comeup, and smooths out the comedown).

However, if you also have GAD, I'd suggest maybe trying Dexedrine IR before going back to Adderall IR. Might need a slight increase in dosage for the same effects, but it's much easier on the body and you can definitely tell. Adderall can cause sudden onset mood swings too and more anxiogenic than dexedrine, because of the l-amp in the mixture.

Thank you everyone for the feedback. Woke up at 4:50 AM with a killer headache and couldn't go back to sleep. Also had some trouble falling asleep. Note to self: never take these meds together again!
 
Pharma-Sutra, my doc and I discussed the downside of XR meds and that's why we went with the Adderral IR. we chose adderall bc I've taken it in the past and it works, but it does mess with my GAD. I haven't ever heard of Dexedrine IR. What is it exactly?
 
He was referring to Ritalin sustained release, which is the exact same thing as Concerta, both are methylphenidate time-release.

Adderall is in part dextroamphetamine (Dexedrine) so you have nothing to worry about in that respect. I think you've made the right choice choosing Adderall over Concerta, as it's superior in every department.
 
another good ADD med is vyvance it's now being prescribed for add. My son was on them, and so was I for a bit. He didn't like them because he just couldn't eat while taking them, but I thought they were much "cleaner" than stuff like ritilan and adderal (which I am back on again).
Some meds work for some folks, and not others..but it's nice in this day and age to have choices.

Actually sustain-release has a different mechanism than the extended-release, well at least in the case of methylphenidate medications.

Basically, the Ritalin LA produces a bi-modal plasma concentration-time profile with two distinct peaks (approx. 4 hours apart, with the first peak within 30 min of ingestion like with IR). It has the exact same release mechanism of Concerta. Thus one can feel like the experience is like a roller-coaster and that is why a lot people prefer a more consistent medication. For someone like me with high metabolism, Concerta and Ritalin LA can only last up to approx. 4-5 hrs at most.

Ritalin SR tablets feels more gradual (instead of two peaks, you get more of a prolonged peak plasma), because it is absorbed extremely well by the body like regular IR tablets.
Ritalin-SR tablets can have duration up to approx. 8 hrs
" Relative bioavailability of the SR tablet compared to the Ritalin tablet, measured by
the urinary excretion of Ritalin major metabolite (α-phenyl-2-piperidine acetic acid) was
105% (49%-168% ) in children and 101% (85%-152% ) in adults. The time to peak rate in
children was 4.7 hours (1.3-8.2 hours) for the SR tablets and 1.9 hours (0.3-4.4 hours) for the
tablets. An average of 67% of SR tablet dose was excreted in children as compared to 86% in
adults. "

And if you have Generalized Anxiety Disorder and panic attacks, steer away from Adderall. The levo-amp in the salt mixture is the main contributor to the many peripheral effects of the medication. It wouldn't hurt to ask your doctor about Dexedrine IR, which only contains the d-amp, the active component that alleviates ADD/ADHD symptoms. Essentially, Adderall has 3.75mg of d-mp in their 5mg IR tablets. You can then calculate what the proper dosage if you decide to test out the Dexedrine.
Although, I'd also recommend sampling the Ritalin-SR first. Great efficacy on enhancing cognitive ability, but like all mph medications, it's better utilized for getting a lot of work done that requires empirical knowledge (like math, doing accounting work, etc). While Dexedrine is like the relaxed, chilled out relative of Adderall, and thus have higher potential for abuse. Dexedrine can make you more extroverted and talkative too, a great confidence booster.

And I should add, Klonopin (Clonazepam) that you take for GAD is probably what is responsible for the lethargic/tired/sleepy feeling you that you got (coincidentally the same time you trialed with Concerta). How much do you take on a daily basis? You should tell your doctor about this, he/she might suggest you to split the dosage or take them only when necessary. If it were me, I would take the kpins later on, long after the my ADD meds have worn off to counteract the anxious feelings from a stimulant comedown. This way you can utilize both medications without dulling one another. OR half the prescribed dosage when you start to feel uncomfortable during the peak of your ADD meds.
