A friend of mine just got permanent brain and heart damage by... a tryptamine?

But the friend who ingested the 4-HO-MET seems to have clear cognitive changes due to this trip according to what the OP has said.

Could be as much of a spiritual crisis as a physical one.

Just saying, don't leave it up to doctors to tell you what went wrong during your bad trip.

Most likely he'll be much better within a week or so...

It very well could be that the psychedelic drug brought to the forefront a mental condition that had no yet manifested itself. My brother's friend was never the same person after eating a vial of acid, even months later he was "different". Don't know if you call it brain damage, brain dysfunction or what, but something's different. I assume the same is true here.

Sorry but request for information; your friend who was by all accounts "forced into an ambulance" was subjected to MRI's of his brain and heart, which I can tell you are not routine drug psychosis diagnostics.

What else happend?
Perhaps a cardiac arrest would explain some of these occurances?

We are feeling the wall for the light switch i fear...
