Opioids MGM-15 / 7-OH the unique inefficacy of Bupe for cravings and blockade. I need your help.

TL/DR: With respect to MGM-15 and 7-OH, can we please discuss how to control the uniquely strong emotional pull / compulsion to use this particular sub-class of semi-synthetic opioids for which 24-mg of SL buprenorphine provides at best a trivial blockade of 60-mg of MGM-15? I know many will disagree that the compulsion is not strong, for many very experienced opioid users it is, let’s not debate an aspect that often devolves into a shaming exercise. I am not opposed to 12-step/AA/abstinence based recovery but they do not work for myself, please and thank you for not going there today. I need an opioid on board or I will get high on another.

I am looking to discuss, for the community’s benefit and mine as well, any solutions using medicated assisted recovery (MAT) with FDA approved or well known RC mu opioid antagonists, agonists and mixed mode opioids - not supplements, clonidine, BSO, benzos, etc. I have and use all of those to manage inter-dose withdrawal discomfort. I am interested in the SR-xxxxx compounds but since I have proven treatment resistant without complete "24/7 365” blockade of effects I am not sure the SR- compounds are sustainable. I cannot seem to get by without blockade. If I think I can get high I will try.

Based on CoAs and competing products I estimate I consume about 120 mg of MGM-15 every 24 hours when I am abusing it, which is more days than not lately.

I know that was long for a TL/DR, thank you for reading that much. I am very grateful for this community. The above is complete enough for proper replies, the following is background and detail, mostly.

I would post this to the recovery forum but I feel it best to start here and wish to help others who are 7-OH curious or current users who are in a similar spot, in time I will use the recovery sub-forum. I tried to keep this brief, but please for me this is an urgent topic not a debate. I am a decades long member of the BL community, I had to create a new user a few years ago but I’ve been on BL since ca. 2002.

I am a former IV black tar heroin abuser (early 2000s) then was on methadone for 5 years, followed by bupe starting in the 2010s. I continue to take bupe but not on some days I abuse kratom alkaloids, but I have taken bupe at least every 36 hours, my usual bupe dose is 3-mg intranasal (~6-mg SL eq.), as subutex generic (no naloxone). I have had no issues with PWD. I’ve used and abused most all the traditional compounds outside of the deliriant and dissociative classes. This is approximately my 313th rodeo, I am 60yrs old, a U.S. citizen and I live in a fairly liberal state. I have a good life, a stable life.

I am writing today in the hopes of maintaining my pre-kraton alkaloids lifestyle, as it was from several years until June of 2025. I am and apparently always will be an opioid addict. I am not opposed to 12-step/AA/abstinence based recovery but they do not work for myself. I can control alcohol, cannabis, psychedelics and I can avoid cocaine well enough I haven’t tried to access it and have denied it consistently for the past four years. I find MAT the best solution for myself, however bupe does not block MGM-15 or 7-OH to a degree sufficient to cease my use. I can easily avoid (semi-)synthetic analogs of everything I have tried except mitragynine. I do not use fentanyl or the post-heroin opioid options. I like to breathe.

The ease of access is a big problem

Is my only realistic long term option methadone? I will require it long term, I haven’t been without at least a partial agonist since 2000. I have several years of past experience with methadone from clinics but clinics in my area object to my prescribed benzo use which is non-negotiable or would take years to discontinue. I have tried up to 32-mg SL eq. of bupe and there is a blunting of effects but it is not enough.

The compulsion to use is in my experience is overwhelming. I have tried to stop using every week for three months. I had a grip on my use until about early March of this year, but even then was unhappy I felt a strong compulsion to use 7-OH. Everyone is unique, many people feel no pull after using 7-OH, even after abusing it. However, the anecdotal data is impossible to discount and I concur - for some people like myself these compounds are as difficult to discontinue long term as others experience with alcohol, heroin or cocaine. A pattern of controlled use is not desired, I don not enjoy them enough.

If black tar heroin reappeared I would be thrilled but also able to control use b/c bupe fully blocks it for me, this was true from 2008 to 2016 when BTH (essentially) disappeared from my region. The pull of kratom alkaloids that override bupe for me is insane. It is the compulsion and the acute abstinence syndrome I experience - extreme lethargy, anhedonia, etc. that get me. These are, ime symptoms more associated with semi-synthetic thebaine based opioid PAW that get me. With MGM-15 it is often within 12 to 36 hours of my last dose I feel compelled to get more (as soon as I feel energy enough to leave the house). I will not order it online, I can resist that but the local smoke shops I cannot avoid. I can well afford my habit, it’s not money, it’s my lifestyle I desire to reclaim.

These compounds are available 24/7 within a few miles of where I reside. I live alone and have nobody to hold me to account and I do not want a sponsor as my main support, thank you for understanding I need to fully exhaust the MAT route before I try abstinence based treatment again. Drug testing / UAs would be fine with me, I can be accountable to a system but not a person or higher being with no formal training in MAT. I don't want to debate this until MAT is exhausted, as an adjunct - assume I am already there.

I am one of the people who cannot reasonably control usage of these two compounds, for myself the emotional pull - the compulsion to use them is on par with my 2022 three month run with freebase and my more distant, two year use (early 2000s) of IV cocaine. I know many people will read that sentence as hyperbolic and push back hard claiming there is no way 7-OH can pull one in as hard as freebase cocaine. I get that sentiment. I felt that way a year ago. Let’s just sort that out now - for at least one person with vast experience with cocaine, heroin and kratom alkaloids the pull of the latter is strong enough to compromise my lifestyle to a significant degree.

The main point I’d like this community to attempt to address and to refine as time elapses, is how to maintain abstinence in the face of the inefficacy of buprenorphine to block these compounds to any significant degree.

Is methadone the only medicine based therapy that would give me both craving control and blockade of the effects? Would naltrexone work as well? Methadone is available to me but at a high social and economic cost. I am nearing the point of paying those costs, but I take a prescribed benzo and would be denied take home doses by the clinics in my area.

I have been an opiate warrior for decades and yes the pull is mentally strong with 7-oh unlike I have had before and I think it is because it can chew through a suboxone regiment like it was nothing.
 
May I ask why you wont order online and instead spend so much more for worse quality at the smoke shops? Have you tried preloading and tapering with SR yet? I am in a similar situation, but I find ordering online and having a dosing regimen makes it all much more manageable. I am not in a situation where I want to discontinue use yet, I find these alkaloids very helpful for my mental health issues. I am on the Brixadi shot, but I am also discontinuing that. My last injection was two months ago. If you want to chat you can DM me. I do know a lot about these alkaloids and I also have some resources and support groups you could access that are about quitting these substances. Sorry you're having a hard time. And I just ask the first question because it's just really surprising to me you'd only want to use smoke shops with their sketchy products VS clean, pure lab tested products. Blessings friend.

oh man i recognize so much of these feelings. i was just about to write a post- which i may still do- when i saw this.

the emotional pull is strong, especially i think if you already have mental health issues...and i mean Any mental health struggles, not just serious diagnosed disorders. i've had depression and anxiety (panic disorder with depression) which started in my mid 20's. i've been on kratom of some kind since mid year 2022. i went from kratom powder (usually red vein), to extracts, to 7oh, to pseudo (when they took 7oh away), to mgm15...and now to suptavex cause they just got mgm15 out of florida. apparently there is a next generation mgm15 coming out at the end of this month, says my smoke shop guy.

the thing is, i'm experiencing more depression and anxiety than i ever have in my life. it's a stressful time right now and my living arrangement is kinda up in the air. i Just got a good job so now i can start saving up money to move to a more stable place- like my own apartment. i'm battling ovarian cancer at the same time as all this and going through chemo. so to say it's a stressful time is a HUGE understatement. but... over the last 4 or 5 months, i feel like i've lost a huge part of me.
[just to give you an idea, i was taking about 120-150mg of mgm15/day. i would usually just take the whole dose in the morning and be good all day. i was fine with kratom powder, to take it or leave it. i would take it once a day or 3 times a day, whenever i had it. but it wasn't until i switched to 7oh ,because it was a new thing in the shop and i didn't have enough to get regular extract, that i started to feel the physical addiction. as soon as i start 7oh, it all got away from me and within 1 week of using it, i was dependent. and i haven't stopped until now. and i'm REALLY struggling. it's been about 5 days since i've had mgm15. i've been taking easy septavex to ease SOME of the wd- like diarrhea, but nothing is making me feel better. i'm on edge from the time i wake up, which is too early, till the time i reluctantly close my eyes. i feel like i'm not myself. i don't enjoy anything at all. every thing is stressful and i can't relax to save my life.

if anyone has any advice, i would absolutely appreciate that and open my mind to it. please, i really need help. i am miserable.
 
I "relapsed" on a few toss & wash scoops of powdered leaf kratom, not sure really how many grams, maybe 2.

Anyway it wrecked me(as in I hadn'tfelt 'sick' for a long while and this put me into abouta weeks worth of withdrawal), and realized I will never really touch it unless I am in dire straights.

But yeah i always noticed how I could use kratom or 70h on top of my regular suboxone dose((Or fentanyl or tramadol))(which usually I had vicodin or oxy stacked onto this; waste or not) to up my high or more pain relief..so i suspect it is the MANY alkoloids inside kratom & it's extracts.

If you want opioids then find real ones that actually work extremely well and get high because this kratom shit is 2 hours and then shitting out plant matter one shart at a time. Edit: and i suspect messing up our tolerance as well!

My advice honestly is to talk to your doctors and your street dealer for some hydrocodone or codiene.

I don't mean start a habit but ANYONE with a serious 70h or kratom habit WILL find relief from kratom craving and withdrawal hell, and then TAPER(Possibly volumetric dose it) whatever opioid you switched to....my tummy could never hsndle a solid week of dosing heavily on kratom, my stomach always thanked me for even a pinch of something different lol.. but just remember use it forcthe acute phase OR be ready to cold turkey your hydro/codiene.....easiest way out with the least mental and physical anguish in my opinion sorry.

I can easily get pain meds. i don't have a bottle now, but i can easily get a bottle of hydro 5's. i'm sure i could get even stronger if i asked. i could try to use the pills to taper. but i'm concerned with the level of acetamenaphen in them. they are 5/325. i can try to see a dr here at this free clinic, but i have no clue if they do MAT. i've never been a big fan of MAT...espeically when it carries on for a few years. i know way too many people on a steady dose of subs and have been for YEARs. i don't want that, but i also don't wanna feel like this anymore. it's absolutely terrible. i feel lonely and alone and antisocial, way more than usual, and empty and lost.

i should have added this to what i wrote, but the depression/low/despair i feel now i've Never felt in my life. i've dealt with depression since my mid 20's, i would venture to say actually my whole life in some regard, but this kind of anxiety and depression is a monster. you're never really ok. it's always in the back of your mind. it's like i'm impatient waiting for each minute to go by. all the time in front of me seems like HUGE mountains. every minute feels like hours, like an eternity. and i'm just in my mind, suffering. i can't seem to stop. i don't see a silver lining, or even beyond the rain. usually i'll have a few days of this and then i'll go back to normal until it shows up again. or normally if stress keeps building and building, i eventually snap and lash out or get upset, like normal people. but lately, over the last few months, i'm always a 9 out of 10. any tiny, little thing can push me over the edge. i feel like an empty shell, like i'm lost so much of myself. or like someone reached inside my window and turned the light down. i can't shine as bright. i don't even see a point. everyone else seems to be able to wake up and smile and go to work and take care of themselves and wash their dogs or wash their car, and laugh. and take some enjoyment out of their existence. i can't seem to do any of that right now. and i'm not sure why. it has to have something to do with what i'm using. .... but let me say this also....this is not my first rodeo....

i've been clean from heroin since december 2018. heroin was easier to stop than this shit. i went through 4 days of hell and then i started to feel better. it was the skin crawling and vomiting and diarrhea and hot and then cold and then cold sweat and sneezing, watering eyes, runny nose, restlessness, anxiety, sleeplessness, wired feeling, etc. after 4 days, it was over. the longest i've gone since i started this mid-2022 is maybe close to a week without anything. and i got sick. this WD period seems to last a whole lot longer. maybe weeks and weeks. i'm having trouble handling that. and also, when i stopped heroin, it was CT. i was using from 1-2 g per day along with dilaudid pills. and sometimes benzos, but not physically dependent. when i stopped heroin, i started drinking heavily every day: vokda. i stopped heroin xmas eve 2018, and was phsyically addicted to alcohol by march 2019 i would say. then it took me about a year and a half to stop the alcohol. i bascially exchanged one bad habit for another. but the alcohol addiction CAN actually kill you, that's the difference. that's why alcohol, i think, is worse than all these other substances combined.

put it this way, i didn't even know one could feel This bad. i didn't even know this feeling existed. i've isolated myself. i'm living in an area that's pretty rural. i have no friends near me. my family is hours away. my mom is about 4 hours away. i've lost my support system for the most part. had a falling out with some family/friends who were integral in my life.

and now i've stopped talking. i don't tell anyone what's going on with me mentally. i talk about my chemo treatments and how i physically feel, but i can't seem to discuss anything internal wtih anyone. because i don't wanna talk to anyone. i have nothing to offer. my cup is so full, i can barely see over it.

i think i've rambled long enough. this is my favorite thing about bluelight, i can ramble on for pages and pages, and someone will read it. some One.
bluelight is one of the last frontiers for actual expression. and acceptance and community. bluelight has been a game-changer for me. at one time, i was a mod for DP (drug policy). i've spent a lot of time on BL at different times. i've been kind of in an Off phase, but i think i need to change that. maybe if i'm isolated in real life, i can make up for it here. communication and connection are so important.

...let me add one more thing. the way i feel. i would like to talk to someone who has felt this way before. i feel like 80% of me is dead inside. like i've lost my will to thrive. parts of me aren't there anymore. my joy especially. i can't laugh. i have resting worried face (as my aunt used to say).. something just isn't right. i'm not all there right now. can someone help me understand this and put it together. i'm so lost..

i think i've rambled long enough. this is my favorite thing about bluelight, i can ramble on for pages and pages, and someone will read it. some One.
bluelight is one of the last frontiers for actual expression. and acceptance and community. bluelight has been a game-changer for me. at one time, i was a mod for DP (drug policy). i've spent a lot of time on BL at different times. i've been kind of in an Off phase, but i think i need to change that. maybe if i'm isolated in real life, i can make up for it here. communication and connection are so important.

...let me add one more thing. the way i feel. i would like to talk to someone who has felt this way before. i feel like 80% of me is dead inside. like i've lost my will to thrive. parts of me aren't there anymore. my joy especially. i can't laugh. i have resting worried face (as my aunt used to say).. something just isn't right. i'm not all there right now. can someone help me understand this and put it together. i'm so lost..
I read your words and I like to read longer than usual comments..sometimes i write them too.
Your situation is very sad mostly because of your health problems and because of social isolation.
I neved been through chemo but i had some health issues and still have. And I also lost almost all my friends. My friends are now online ( probably 2-3 persons ), my parents because right now I live with them after the end of my 7 years lasting relationship. And my pets - one cat and one dog.
The idea of finding social connection online is actually good, it is great if u find right people. I will check Bluelight every day and i will be here for you. Try to find other people here, through some online game, facebook for example is not that good for finding people who will provide you will what you need right now. And that is honest and no matter how long response to your questions, feelings, just response to anything u share with us.
Loneliness can be very uncomfortable and sad. Not always but right now I like to connect with other people because so many friendships ended...and it made me sad, angry, disappointed...I coudn't sleep because of it but at the same time i am so tired...

Something positive i can tell you right now..
Do what is best for you - eat the best and most healthy food u can get, drink clean water, or tea, coffee ( don't overdose on caffeine ). Clean your place. Maybe take care of some plants, later for pet. All of this will help you overcoming the time which is in front of you ( and in front of me ) and which is required for your body and mind to heel.
For example, i quit many meds and stim use and i know i wont be ok for some time, months maybe year or more. But eventually i know the goal i have - to be healthy and not so tired and depressed will be reached and every good decision is helping me to reach it faster.

I have diarrhea even now, not physically addicted to opioids but i still use them like 4 days out of 14. I still have insomnia, diarrhea which i can't figure out the reason why i have diarrhea...Time is going by super slowly...
It's good to white a long comment and to obtain a long response...But I will provide you with more helpful responses later, consider this as an intro.
U will get through your struggle and your life can be balanced, peaceful and filled by little joy of small daily things which will make you smile. Because of happiness which u will feel again.
 
Why don't you try upping your bupe dose? Seems like you are taking barely any, so maybe that is why it doesn't work for you. Personally though if you feel you will be on opioids the rest of your life methadone is the best option. It will control WDs and cravings once you get to the right dose (don't stop at an arbitrary dose like 100 mg, just go up til you feel ok),and then if you want to get high of kratom or something every so often, you still can. I am the same way, I think I will need to be on opiates for the rest of my life to function properly and I'm on methadone. Occasionally I will get high off fentanyl and it still works, even just 1-2 pills will get me high. So I think methadone is the best idea.

There won't be any physical one except of being super tired - but i contribute this to quitting uppers. But also after quitting benzos i am more tired than i was while taking them.
Now I get bad itching from dihydrocodeine and it makes me more awake.

Quitting uppers can make you incredibly tired for a long time! And tbh I do not get any side effects from the methadone aside from constipation which can be easily dealt with through eating fresh produce and fiber and drinking a lot of fluids. But itching from dhc you can address with any antihistamine.
 
Both ligands produces tachyphylaxis i.e. the dose required to produces the sought effects increases quickly. But at the same time, tolarance builds so just to feel 'normal' you have ro redose more often as well.

I only read about this specific issue dubbed 'the zero trap' in a few recent papers.

could you maybe please share those papers with me? this is one part of kratom alkaloids that i find incredibly fascinating and i wasnt aware they already started looking into it. Would be much appreciated to be able to read those

U tried cetirizine, levocetirixine, desliratadine, loratadine, diphenhydramine, promethazine, hydroxizine, even fu*king chlirprothixen and mirtazapine. And combinations of high doses of these mentioned ( i tried more of them but i don't remember what were they called ).
Nothing helped, it only made high difficulty insane itchihg just medium- insane itching. I had to use lower doses of dhc. Never had methadone but even hydromorphone caused much less itching than dihydrocodeine.

If antihistamines don't work for itching, I wonder if steroids might? Oral steroids in particular. Codeine is notorious for it's histamine release though (one reason why it is not administered IV) so it makes sense that dihydrocodeine would be that way, too. It isn't any surprise to me that hydromorphone didn't cause much itching, though, I don't [think I caught an itch from it even when shooting.
