Oxymorphone is powerful as fuck when you snort it. I almost ODed on it multiple times when the old school ERs were still common.

Back when those octagonal Opana 40 tablets were still prescribed, every single person who tried the stuff loved it BUT whereas oxycodone is MORE potent if consumed orally, oxymorphone is around x5 more potent if consumed parentherally.

Oxymorphone is almost exactly like the stuff that used to be prescribed in the UK. It FAVOURS abuse.

Maybe it's purely coincidental, but it's potency (both oral and parentheral) matches oxymorphone.

But as I noted, I found an image of the original 1950s pamphlet posted to GPs in the UK where 'high oral bioavailability' was a 'key advantage' so unless a doctor had good reason to obtain the academic paper (a long job in those days), they might reasonably believe that people consuming the pills via a parentheral route was unlikely. But a straw-poll suggests that back when it was quite commonly prescribed, it killed almost everyone who DID abuse them.

I should add that even orally, the stuff was considered inconsistant by users. The classic comment was 'one day you swallow five and are fine, the next day you swallow three and turn blue'.

But as I said, this stuff is now thankfully NOT in the BNF. How many had to die for the dangers to be acknowledged, I have no idea. But certainly for over a decade the stuff was infamous...

Hydromorphone (Dilaudid) IS no doubt a Tier 1 Opioid, slightly more potent than Diacetylmorphine (Heroin) IV administration, Dilaudid is AMAZINGLY Potent & Euphoric…….oral administration is despicable with first pass metabolism destroying 70% of dose by the liver, Hydromorphone MUST be administered IV/IM/SC injection, not crushed oral tablets but genuine pharmaceutical vials.

I never was a fan of Dilaudid. One time I had about 10 of the 8mg pills and was IVing them and all I got out of it was a milli-second rush following feeling extremely sober. I ran through all of the pills in one sitting and was very upset. I was IVing heroin at the time so I had a tolerance but still if Dilaudid is supposedly so powerful I should of been able to feel it.

Dilly was always my fav opiate but way to addictive. I got to the point where i was shooting 3 18mg hydromorph contins every 4 hours or so

I've had mixed results with Dilaudid. I'm not an IV user so maybe that's why. I never sniffed my pills either until I realized they were absorbed better than orally. I did try insufflation & it did get me high on the second day I used that way but I think I did a bit too much & couldn't get out of bed the next day. There was no rush , no euphoria, just uneventful, well I guess I'm high feeling. I can get an 8mg for $10 but I probably won't even bother again.
Sniffing just isn't for me.

I need a bit of help, I'm am a super experienced drug user, from opioid pills and cocaine as a teen hustler to fentanyl and crack/cocaine shots as a homeless. I've been through it all. Recently I got sober, I'm two years in with only one or twice use of fentanyl. My goal is to be able to get high (very occasionally) without using fentanyl so I'm working on lowering my tolerance. I currently have kadian(XR morphine) and Dilaudid(hydromorphone) prescribed. Tell me how I can take 50 Dilaudid 8mg pills in one handful and not feel anything????? Nothing at all. And it's not a joke I think my last try was 40 pills. I got so fed up I felt I was wasting my pills like I can get at least 100 usddollars for 40 Dilaudids around here. And when shooting them I can only fit 16 8s into a syringe, the fat ,3ml syringe. And I still feel NOTHING. The liquid becomes SUPER THICK and I was wondering if anybody can teach me how to make a more concentrated Dilaudid liquid iv. Or should I just give up?? It seems like fentanyl ruined my tolerance for opioids 100%. I'm so discouraged and I really wish I could just feel the warm rush like the old days. Does any body else have the same problems I'm having and is there anyway to get around it?? I'm hoping somebody knows how to make a super concentrated liquid iv with Dilaudids and I'll keep trying to lower my tolerance in the mean time. Please help me if you can

If you can't feel 128 mg of hydromorphone IV you should just give up. That's beyond ridiculous. I could feel 8 mg even when I was using a gram of heroin a day.

Hey @Legitjohn First off, major kudos for getting off of the Fentanyl. That is huge. Make sure you give yourself credit for that.

Opioids were my drug of choice. I lived in Cambodia for a while when I was teaching and I had essentially unlimited Heroin for the first time in my life. I could use as much as I wanted and still eat and pay the rent. I got to a point where I was using 3-4g of relatively pure Heroin every day, intravenously. All I felt was numb. I didn't feel good, bad or anything. There was no color, emotion or anything.

I was using higher and higher quantities trying to break through this wall. It only made me feel more numb. I eventually felt trapped. I came to the realization that the only two options were to reduce my tolerance or to keep feeling nothing. I ended up reducing the dosage on my own volition. The only way I would be able to enjoy Opioids again would be to unsaturate my body and mind.

I eventually got to a point where I was using ~1.5g Heroin per day. This is still on the far end in terms of the maximum amount I could use and still receive benefit. I'm pretty sure that if I were to make all of the right decisions, my optimum dosage would be ~500mg - 750mg pure Heroin per day, perhaps even less. You have to find a sweet spot.

Someone really ought to copy/paste a bunch of the best BL drug stories (like this one obviously) into one narrative about a guy... let's call him Colin. Colin Sick. A globetrotting junkie with a million stories from teaching in the far east whilst sniffing rails off a chalkboard to getting in meth-fueled battles with kangaroos and growing weed in the hills of Northern California or something, lol.

Yep, you may be fucked in this regard. Fentanyl and its analogues are so potent that exposure to them has long-lasting, if not permanent, effects on opioid sensitivity.

A few years ago I tried IV hydromorphone at a dose of 16mg, having not had any opioids for like a year and a half. I was really worried that I might kill myself, but shot it up anyway. The result? I got a little bit of an itch, tiny pupils, but that's about it. I could determine the drug was active, but no reinforcing effects. I can also take people's replacement doses of methadone (like, serious doses) without issue.

I had a short period of abusing fentanyl, carfentanil, furanyl-fentanyl as well as a massive overdose of about 100mg fentanyl this one time. I think that might have wired my brain to tolerate these substances to a point where they don't really work.

But yeah, 128mg Dilaudid IV is insane. I also went up to shooting about a gram of dope a day, and that would still have fucked me up. No idea what it would do to me now, and I don't intend to try.

Four(4) Oxycodone 80mg CDN tablets on same spoon….IV administration and didn’t feel SHIT …..a sickening waste of precious opioids…..why? All the mu-opioid receptors were being occupied by large regular concentrations of Methadone HCL so all that Oxycodone had nowhere to bind to…..I probably needed a moderate dose of Carfentanyl to break through the Methadone

That was like 20 years ago…… now Methadone 100-125 daily and I would even try another opioid ……unless I was 3-5 days no Methadone and I had a large dose of Hydromorphone 30-50mg and id oral syringe up my bum bum while laying on the bed…..in 15 seconds that unmistakable WARMTH & Comfort engulfing envy part ….ohhhh that beautiful Opioid narcotic…ohhh Banging real H 20 years ago ohhhh…..Speedballs ohhhh….lol 20 years ago
