I take a small dose of zyprexa (atypical antipsychotic) every night (2.5 - 5 mg) and know that it will dampen the 5HT2A effects of the mdma a bit.
If the zyprexa is taken not on the night of the mdma but only the day before and after (always leaving a 24h space), is there a risk of serotonin syndrom ?

Not serotonin syndrome but it's highly unlikely that you will get much from MDMA even waiting 24 hours and sorry but if you're on Zyprexa then your doctor has deemed your mental health condition to be either treatment resistant or extremely severe and you should not be doing drugs regardless. Skipping your medication is never advised and that particular medication has some nasty withdrawal symptoms.
