Why can tolerance exist isolated without other adverse effects?

When we think of tolerance, it usually comes to increased receptor or transporter density. On top of that, with some drugs like opioids, comes less release of endogenous agonists. These cause withdrawal syndrome but with others, notably nmda antagonists but also to some degree with seroton/dopaminergics it is possible to build heavy tolerance but apparently no other adverse effects when avoiding daily or top frequent use.

How is it possible that a friend is able to take 1g DXM or same amount of morphine (these two seem to be somewhat cross tolerant because of similar structures even when structure shouldnt be relevant for tolerance afaik, but she doesn't lie about the doses and I see no other way for once or twice a week use to remain such high tolerance) without passing out and not getting any impact on sober days?

Is this a metabolic thing, upregulated CYP enzymes or possible at receptor level?
