Hey all,

I'll be starting suboxone in detox when my bed becomes available but I'm worried about the daily pick up once I'm home - it's a 110km trip to the nearest pharmacy/doctor etc!!! What is the soonest you've moved up from daily pick ups? I just want it known I don't plan to misuse it, I want my life back on track I just can't afford to be driving all that distance every day let alone physically being able to do it right now.

Any help greatly appreciated, TIA :)

As a rule it's 2 months until your first TA's (and even then it's only 6/month), but I've heard of people in special circumstances getting them early. This is something you'd have to talk about with whoever is going to prescribe the suboxone.
 
Is an inpatient rehab an option for you, rather than just a detox and transition to suboxone?
 
Hi Spacejunk, not at this stage as I have three kids who have already put up with me being in the looney bin for two months over Christmas and into this new year. So just 10 days has them quite anxious right now and I hate myself for putting them through this. They're 9, 11 & 13 so old enough to know what's going on, though they think my admission is psych medication related (changing meds, so not a straight out lie). When they're old enough I'll divulge my drug use but they're too young right now IMO.

I'm in a similar position. I had to prove my employment and everything which upset me because of the risk of exposure of ultra private information.

Ask for double dosing or even triple dosing.

Get a female case worker and doctor. talk up the kid aspect, work, school, distance.

Above give clean piss. offer to do weekly piss tests. This is what is going to do it for you i.e. get takeaway dosing. Its ironic because takeaway dosing and its withdrawal because of dirty piss would be seen as retaliation/punishment. The NSW Opiate dosing document expressly states that surveillance is not to be used to determine if users should be punished

Though the problem is the clinics don't normally do takeaways. You have to move to a chemist and they won't do that unless you tell them you'll lose your job (and even then it'll be 6 months min). edit: actually i don't want to give you the wrong expectation. I had basically the boss of a clinic promise if i went on the program to give me takeway dosing within 2 weeks. Turned out that the doctors are utter fuckwits. Seriously they'll lie, like really badly. They'll say X and write Y on your script or records. IF they say 4 weeks until takeways they really mean 3 months. They fuck up scripts all the time. Ask to double check. Shit i saw 8 doctors in the first month.

My last three scripts have been screwed. Fuck my doctor told the chemist to give me half a fucking strip. like cut it in half!! Fuck you western sydney health prescribers

It also helps to be really polite, well behaved and always, always, keep the queue. Shit you'll get shanked if you push in.

So probably 12 months for takeways. Unless you're pregnant.

6.4.4 Access issues
So long as there are no contraindications, takeaway doses
above and beyond those normally recommended may be
provided if one or more of the following conditions is
demonstrated:
• work commitments interfere with daily attendance at a
dosing site
• travel hardship — remote area, large distances and or
prohibitive cost of travel to a dosing site make daily
attendance a hardship
• education commitments interfere with daily attendance
at a dosing site
• parenting commitments interfere with daily attendance
at a dosing site
• other factors (which should be documented) interfere
with daily attendance at dosing site.
It is important that the prescriber sight evidence of the condition
justifying increased takeaway doses. This may be in the
form of group certificate, pay statement, or a letter from a
general practitioner, pharmacist or other agency or person
who can confirm the social circumstances of the patient.
In remote areas, opioid treatment services may have to
develop a policy on takeaway doses that acknowledges the
impracticality of 7-day-a-week on-site dosing for some
patients. Such a policy must address the objective of minimising
the diversion and injection of methadone and buprenorphine.
Absolute contraindications cannot be ignored.
Reasons for going outside guidelines should be documented

Thanks bunches Chugs, you've really helped me out here and I appreciate it heaps. I go in on Monday to detox so will go armed with info lol.

There's a pharmacy closer that's going to get a permit to administer it to me or something according to my AOD nurse. 80km trip instead of 110km so a good distance less, still too damn far to be driving each day but I'm greatful :)

I know of pharmacies in Melbourne that will do TAs for suboxone almost immediately - just give a clean piss sample and you're golden. The rules for TAs are a LOT more lax for suboxone than methadone. Methadone is another beast entirely, but I would imagine, given your circumstances, they'll try and be reasonably accommodating - I'd be surprised if you had to wait even a fortnight, to be honest. Speak to your doctor and your chemist about the situation.

All that said, that still sounds like an almighty pain in the ass, even once you get on 5 weekly TAs, that is still a couple 220km round trips a week!

Are you near Melbourne? I'm very recently on suboxone, & changed to subutex tablets after seven days of itching (hopefully I'm allergic to the naltrexone), so I can up my dose from 16-20mgs to 32mgs. My doctor thought I'd already be at this dose & is very open minded about the "ceiling effect".

How did the detox work for you?

I detoxed in three days from a very heavy habit. Suffered to the extreme for ten days, then got in with a private specialist. After just five days I was allowed takeaways from the pharmacy.

Especially as mon morning I returned an extra 4mgs sub that I didn't need over the w/end. To earn points with the pharmacist ;)

wow that's a huge dose - 32mg. Most people I know get utterly maggoted on that much.

This was what i was told that if you want takeways that you can basically pay for a private specialist.

How much did it cost if you don't mind?
 
^Thats the surprising thing, not anymore than a GP appt after Medicare rebate. $35 out of pocket.

Course the travel time is inconvenient, but not as bad as the above poster having to travel that far daily to a pharmacy.

I just could not make any headway through the public system. I couldn't just be drastically tapered off large amounts of opiates & wait for months for a substitute. Where I live there are no gps at all, or even private prescribers taking new patients.

So, I felt no other option then to look to the nearest major city. Actually, the doctor answered the phone as her receptionist hadn't started for the day. She was prepared to see me that very evening, & arranged everything at the first appt.

I do have chronic pain issues, hence larger doses then would be used for purely maintenance are worth trialling, in my case, before if necessary having to move onto the big bad 'done.

I need to be functional, sometimes you get to the point where you just do what you need to do.

alot of users get high by injecting like .2mg. for some reason the lower the dose the higher you get, all to complicated for me. im in 32mg ever 2nd/3rd day but i can go up to 5 without to many withdrawl effects (i just show up wearing 2 tshirts 2 jumpers a puffer jackets and 2 leather jackets, i really feel the cold when withdrawing. i was only on H for a few months (after years of OTC opiate abuse) but i chose to stay on it, it keeps me safe from tempation and i never want to go back to where it took me. I havent been able to get take aways, but thats my fault for consistintly giving bad pee. They said just go a week and give clean urine and you can get T/A but the rest of the story is about that F*#&ing ice epedemic
