Need advice for talking to a doctor

I have a doctor's appointment scheduled for Tuesday morning. This is my old family doctor but I haven't seen him in years. Unfortunately, my med records don't look great from the "drug seeking" perspective.

I am currently prescribed gabapentin and amitriptyline. But I do also take benzos for sleep. I know I'm gonna fail there. However, there is a silver lining. He had me on a kpin script for years. After I finished school I kinda let it go stupid fuckin mistake lol.

So here's the question do I be straight with the doc about what I want and what I need? I am well versed in most pharmas so I could even get a little technical with him.

At the same time I don't want to make myself look like an addict.

My thoughts are to push for a lyrcia script to replace the gabs and then slowly try to get a real sleep med. I've had insomnia since I was 9yrs old. Most weeks I average at least one nights missed sleep and it can get worse than that. There was a time I was up for like 3 days straight and I wasn't on stand or anything just insomnia and when I tried to go to sleep I couldn't. I have to knock my ass out with my pins.

Sorry for the long winded post. But I would love anyone's feedback on this situation. I will say venzos have been one of the only class of drugs that help me shut my brain off.
 
In my opinion it’s always best to play dumb with doctors. They all have a superiority complex and stroking their ego by appearing stupid and uneducated about the drugs you want has helped me secure oxy scripts, benzo scripts etc even with a big red flag on my file for drug seeking. My strategy is just push my issue hard and start fake crying about how bad I feel and act like I hate having to take these meds but it’s the only thing that’s working for me. By appearing like I hate the feeling of being on the meds but need them to function the doctors seem to agree with me and sympathise with me. Basically what I’m doing is deploying a smokescreen to conceal my intentions while using that smokescreen to simeltaneously deploy a rouse. Works a charm every time and I’ve been given like 400 oxycodone 20mgs 2 days after literally drug seeking at the ER.

These doctors are absolute scumbag cunt whores who deserve nothing but suffering for the way they manipulate and judge patients based on their own personal feelings so you play the fuck out of them, wage a silent war on them, and bend them to your will. Play the long game my friend.

Never appear "well versed in most pharmas", or they'll figure you're a druggy.
Just tell him you're getting a new doctor for whatever reason and you remembered him as good. Then tell him what you're currently prescribed. Once you get that in place, mention trouble sleeping and how the kpins he used to prescribe helped that a lot. If you get that, instead of having to try a Z drug, be still and go home. Work on the Lyrica substitution later down the road if you really want to.

My bet is that he will take over your refills and you will have to jump through some hoops with a Z drug for a while to end up on a benzo.
But, one thing at a time and take it slow grasshopper.

It's hard to find a doc who will basically take your advice and listen to you on treatment even if it's not psychoactive drugs.
I used to have a GP who balked on sending me to physical therapy when I told her that's what I wanted. It was a condition which I had seen numerous specialists for years ago, and physical therapy was what was ultimately prescribed by them. She's not my doctor anymore.

There are good ones out there still. Becoming more rare.
What's the difference between a doctor and God? God doesn't think he's a doctor.
 
Just wanted to throw up an update.

Had my appointment this morning and I realized just how lucky I was. When asked about sleep health, meds, etc I ran through the shopping list of meds I have tried. Got a little detailed in how I felt they affected me and my sleep. One small passing comment about how the kpins were one of few drugs that truly help me sleep and the next thing I see on his computer screen is a new prescription for Klonopin.

Dose is definitely on the low side 1mg/per night plus as needed for anxiety. I would preferably be at 2-3mg/night plus as needed. But hey baby steps right? Should I bring up the dose issue sooner or later? As in after the first week on the med say hey it's working but I've noticed I've had to take 2 of them instead of 1 or something along those lines?

I had forgotten how great of a doctor he was. It had been ages but still the same good dude. He actually listened to what I had to say especially related to the cornucopia of sleep meds I have tried. I brought up some of the nasty side effects of Seroquel (previous med) and couple other meds. Instead of dismissal or redirects I got genuine responses and definitely felt him caring about what I had to say and how these things affect my life. I will never let myself stop seeing this man for as long as he's practicing.

Do not I repeat do not go in after a week and tell him you’re taking more than you should. Biggest red flag for a doc is that you aren’t taking it as he’s prescribed. It’s a blessing he’s given you the Klons. I would just try them at the prescribed dose? 1mg of kpin is plenty. If you’re needing 2-3mg just to get to sleep then you’ve got a serious problem my friend.
 
When I was previously prescribed I was at 2mg plus as needed. And the week thing was more of generalization I did not literally mean a week. But I understand what you are saying. Any other doc I've had when it comes to sleep meds have been pretty receptive when it comes to dose changes. But I digress I will wait it out a bit longer.

And no my friend I do not have a problem. Ever since I've taken benzos I've had a weirdly high natural tolerance. I was looking into it an apparently due to my hair color there have been many studies proving that certain hair color correlates to a higher tolerance for most drugs.
 
Say it helps but not quite enough and ask him if it's OK to take two since you were previously prescribed two mg.. It's OK to be aware of and remember things you've been prescribed before.
 
Op - I'm glad your appointment went well, and I second brokedownpalace's suggestion.

Anyone else in the same situation, I'd recommend being honest, conveying what had worked before - IF obtained legally - and just try to communicate as concisely as possible. You either have a knowledgeable, compassionate doctor, or you don't. Only one way to find out.

Btw, I don't lol. Or didn't. Currently searching for a doctor who didn't find his medical degree or his understanding of communication in a dumpster. 😉
 
Glad the appointment went good. I’m the same way, I’ll stress myself out and think of literally every scenario of what could go wrong. It usually goes fine though lol. Your doctor sounds hella cool. I would wait a month before asking for an increase. But that’s just me. Cheers
