IQ Test Score in Percentile Format... Just a question...

I've had an IQ test conducted for me a while back, because it was necessary for my application to university (I have no schooling past grade eight, I was an autodidact; I taught myself).

The results were given to me in percentile form. I'm just wondering how each percentile score converts to a number on the typical IQ scale (100 being average). The test is the WAIS-III Canadian.

For example, what is 99.5th percentile in Full Scale Intelligence Quotient equivalent to numerically on the standard scale?
 
just as an FYI, here's the scoring scheme for IQ:

100 points +/- 15 points * (# of standard deviations you are from the mean)

so if you were two standard deviations above (95th percentile), you would get a score of 130

three standard deviations above (99.7th percentile) corresponds to a score of 145

etc.
 
It should also be noted that there has been gradual drift of scores upward over decades, but the scoring is also periodically recalibrated. Depending on the test, it's sometimes more accurate to compare your score to a mean of 105.

I'm not sure I took an IQ test once and they told me I scored the same as Forrest Gump
