Opioids I need help with Hydromorphone dosing

Greetings all,

I hope all is well with you and yours. Here the deal...

I am prescribed 4mg of Hydromorphone a day, two 2mg tablets. I would like my dose raised just a few milligrams but my doctor may not want to. If I was to switch to Hydromorphone Contin would he kind have to raise the dose as it will be spread out over 12 hours or however long hydromorph contin lasts.

If they last 12 hours that means I will be getting 0.3mg every hour which isn't much I don't think. I am hoping he may give me 6mg or higher.

What do you all think? All answers are greatly appreciated.
