Stimulants Detox Psychiatrist Vyvanse - Suboxone NEED advice. do I need new Doctor?

So I went to detox in Feb 2020 to get off of my perscription of oxycodone. I went into a long dark path of addiction and dependence for oxycodone.

I initially was fine on 8-10mg Suboxone a day. My detox Psychiatrist kept upping my sub doseage more and more and more. Roughly 4/5 months after I had left the detox clinic I was on 22 mg Suboxone a day... She started to slowly taper my diazepam (started at 30 mg a day) and after 6 months I was down to 10mg a day...

About 3 or 4 months ago, she diagnosed me with ADD and perscribed me Vyvanse. Due to lockdown and covid, I didn't go into the office for 2 months after first being prescribed Vyvanse.

First time I went in was 1.5 months ago. I tested positive for amphetamine. (Which the urine screening guy named dan told me would happen. But he hasn't been there since the lockdown for some reason. So my psych and addiction detox dr does it) and she told me that I tested positive and has to take away a Suboxone carry. She said "there's a small chance it could be Vyvanse so I will send it into the lab for testing and i will see you in a month"

Well last week I went in.. I gave another urine sample. And I tested positive for amphetamine.

When I went into her office to talk to her she said I was positive for amphetamine again. I said well yea that's because you prescribed me Vyvanse. She said "the lab results are right here. Have u been getting laced weed or anything? You also have benzos in your system" I said no... I don't buy weed or smoke it. I sometimes have edibles that are made from home grown plants that I grew for my father's cancer. Nothing is laced. And YOU PERSCRIBE ME DIAZEPAM YOU HAVE THE ENTIRE TIME.

She said "it says amphetamine... Not Vyvanse.. " I looked at the results and said they don't even say what chemicals are there. Did u not get a full screening done? You realize Vyvanse is amphetamine right? Dan even told me this. Where is he? I'm getting very annoyed now, I'm starting to get pissed.

Then she became VERY unprofessional. She said "amphetamine is usually meth. How do you feel when you smoke meth? Has you grandfather passing away recently given you more cravings?"

I lost my control at being calm. I said "IVE NEVER DONE METH IN MY ENTIRE LIFE. IVE NEVER USED A NEEDLE. IVE NEVER SMOKED ANY DRUG. ID BET MY LIFE ON IT"

She got loud and higher pitched voice and slammed her finger down pointing to lab results that said "amphetamine" and said "WELL I WOULDNT, BECAUSE IT SAYS SO RIGHTTTTT HERE!"

I pulled out my phone and said "hey google..... Does Vyvanse test positive for ampetamine?" And you guys already know what it said

She said "well most patients I prescribe Vyvanse to does not test positive for Vyvanse Everytime!"

I said "this is the 2nd time you've seen me since.... Do you not know how to do your job? We just went into stricter lockdown and you want me to go to the pharmacy every more every week? I've been off of pain medication for over a year now. Every time I come here you say when I come in next time my carried will increase to 5 a week. And u always seem to come up with some BS. I had to ask you two months in a row just to have my Suboxone doseage lowed by 2mg because I am supposed to eventually be off of everything. This is ridiculous!"

She said " come back in three weeks... If you are clean, I will give you back the carry I am taking away and also give you one extra making it 5"
I said can I come tomorrow? Like call Dan the guy that's supposed to do the urine testing. Call the lab. I am taking the medication you prescribe that's why I'm positive for it. I can't believe this."
She said "no"
I said ok can I come next Monday then!? I'm so annoyed with you. Thai is insane. Noone has EVER accused me of smoking meth. Let alone an addiction specialist and Psychiatrist..."



So I'm going tomorrow.. I will obviously test positive again because that's what's supposed to happen. Today when I went to pharmacy, I told the pharmacist what's going on... He said "well if her patients aren't testing positive everytime that's because they aren't taking it... Wow. I'm sorry you're going through this. Hopefully it's all sorted tomorrow"
So in you're guys experience... Is this normal behavior from the dr you see once you leave a detox clinic (over a year later as well) or those of you perscribed Vyvanse, do you test positive all the time for ampetamine? Idk what I should do here ....



TL:DR
Psychiatrist that I've seen since I left detox clinic in Feb 2020 is saying I'm smoking meth, because I've tested positive for amphetamine the last two urine tests since if been prescribed Vyvanse. Which is supposed to be amphetamine. She's taking away Suboxone carries because of it... She increased Suboxone dose for months and months until 2mg away from legit dose. I had to ask for 2 months to get my dose lowered. What's your experience with this? Should I get a diff doctor? This is insane!

 
Threaten to report her to the medical board for malpractice. Print out a scholarly article on vyvanse causing a positive drug screen for amphetamine, go in calmly at first and apologize even though you are not wrong. Tell her it’s not OK for her to play with your prescriptions as the meds you are on are not to be messed with or abruptly stopped, and if she does not comply threaten to report her for malpractice.

Only problem is is if you take it there she may drop you as a patient, but that would be unethical and give you more of a reason to file a report.

What a moron. I got furious just reading this. Don't know what you should do about it however. Is she young? She sounds like an idiot.
 
Unbelievable... And she's a psychiatrist specialized in addiction? One can only wonder what other mistakes she has made, sounds like she's not qualified for the job.
To me that whole situation is unforgivable, specially since she's working with very vulnerable people.
There should be at least some consequences for her malpractice, she needs to acknowledge her mistake and learn from it.


@OpiateKiller is right, I think the first thing to do is to stay as calm as possible and explain the whole situation to her. If you start getting frustrated she will probably get very defensive and refuse to listen, which usually complicates things...
 
Are your tests being sent out for mass spec in a lab or are they doing this on site? You mentioned at one point " she " sent it to a lab almost as an afterthought. If you are just getting a cup test ( dipstick style ) I would request all your tests go out for mass spec. @MsDiz will be of some help.
 
Wow. Your doctor is astoundingly incompetent. I'd not just threaten but go ahead and report her to the board. I know lockdown will make it harder but I would start very earnestly looking for a new doctor if possible.

It sounds like you might be from the UK or Australia talking about bupe take homes, iirc they do it over there (they don't in the US) so I don't know how that works with NHS (or doesn't work—this might be one of the cases of socialized medicine fucking patients over) but that might give you an additional place to complain to. What she's doing is unacceptable and suggests that she doesn't know what she's doing to a dangerous degree. Good luck and sorry you have to go through that nonsense.

Bad healthcare providers in general piss me off not just on an ordinary level but also because they make all of us look bad and, not only that, and this is an even greater problem in psychiatry, one bad experience, even from a provider having a bad day, is enough to turn a person off totally to getting help. We are supposed to do better than that and be held to a higher standard and it sounds like you are truly dealing with the bottom of the barrel.

Unfortunately a lot of bad docs find their way into addiction treatment as just writing bupe Rx all day is seen as easy and low risk money. It is a shame and hurts patients badly 😢
