Is it used clinically where you are? Not surprising methadone works well, given its a weakish NMDA antagonist also.

You might try combining with memantine as an adjunct. It really stamps down hard on tolerance too, both what you have, and what the opioids have in mind for you. Although be careful, its a combination that used right/wrong (depending on who, how much of each and what, why etc. can pack an arse kicking. Not necessarily a bad arse kicking, but an arse and a foot up it nevertheless), I should imagine that it would help with hyperalgesia too.
