Are artery shots always painful?

A friend was shooting something and about 4 hours later they had little red dots, kinda looks like Petechiae. He done it in the crook of his arm, I've always heard artery shots hurt like hell, but they are worried. Has anyone else ever have little reddish, dark reddish bumps after IV use? There isn't any visible swelling, and I checked the cap refill on their fingernails and hands and that was good to go. It goes down the the right side of his right fore arm. He thinks it could have been an artery shot, but he says it didn't hurt at all there are no signs of visible swelling or anything.
