Bupe Tolerance Question??Was prescribed Sub for 1 month. PM Dr putting me back on Morphine

Hi. I have went to the same pain management doctor for over 2 years now. Last month I wasn't able to pay for my visit. (I have no insurance, thanks Obama.) so my Doctor was able to prescribe me Suboxone 8/2mg (60) twice a day... well. I have been taking them twice a day. I have heard that suboxone raises your tolerance. i have an appointment tuesday. I have the money to go. I was on 90 ms contin 60mgs a month. my first question is, since taking 1 1/2 to 2 strips a day for the past 28 days, is my tolerance to opiates like morphine, oxycodone, hydromorphone higher? I work at a place that demands hard labor and come home crying every day in pain since not having money last month to fill my morphine. My doctor believes in Extended release medications. which i find that ms contin works so good for pain. But.. it's very expensive. However my doctor has made an exception for me and told me i can get prescribed instant release or extended release , whatever is cheaper. but for three months only which is fine because i will have money from work saved up. nstant release oxycodone, morphine, hydromorphone are all cheap price wise. Does anyone know what opiate/opioid would work the best for a person (me) switches from being on suboxone for one month, to back to my pain medicine. I would really appreciate it. i hope i'm not breaking any rules. I'm just very emotional and depressed as i am 19 years old. And i have so many back problems it's not funny. I can't even walk right now as i just got off of work.

PS. Any tips: for example should i stop taking the suboxone a few days before i go to my appointment tuesday. Would that lower my tolerance? Sorry, i'm just very scared that whatever opiate pain medicine she writes won't work bcuz of my tolerance now. Anyways thanks for the help.

You may be in for some complications. Buprenorphine is an extremely potent opioid despite being a partial agonist and 16mg of Subs a day is a pretty large dose, probably way too much.
So your tolerance is very likely very high and lots of people experience that normal opioids don't work that good after being on Subs.

I dont want to scare you but it maybe that you need a lot of painkillers. 16mg of Bupre equal about ~400mg of Morphine.

In my opinion your doc fucked you. Bupre for pain is given out in 0,2-0,8mg doses as Temgesic but I don't know about your medical history.

Taper the Subs as far as you can is my best advise. Before you make the switch.

Thanks for the info. I thought my tolerance would go up. I will be going to my appointment Tuesday (July 19) I don't want to go to my doctor in withdrawal. but.. I need my pain medicine to work. And for me the suboxone wasn't working when i took a half. i would still be in withdrawal and did nothing for my pain. scoliosis, spondylolisthesis, spinal stenosis, sciatica, degenerative disk disease. those are just a few.. basically i was born with scoliosis and in 2008 (age 11) started taking Vicoprofen (Hydrocodone/Ibuprofen 7.5/200) for years until i was 15 I was jumped 3 times at school by 3-5 different guys. which caused the diagnosis i listed besides scoliosis. and was referred out to my pain management practice.
- Med info: Prefer Morphine ER for my pain. I have been prescribed [90] 60mg tablets every month for about 1 year. my doctor has only tried Oxycodone IR 20MG for my chronic pain. Which is so much cheaper than ms contin aka morphine. for 3 months im allowed instant release or E.R. whichever is cheaper like i said. Anyways i hope this helps. And my doctor is open to writing anything pretty much for my chronic pain. My biggest fear is the morphine not working. so if you or anyone has any suggestions on what will help the best from switching from suboxone for a month back to my pain meds. Also what day should i stop taking the suboxone? I go exactly a week away. also any tolerance lowering tips. Sorry if i'm blabbering on it's just i go back to work the day after i go to my doctor. scared of pain man.
 
While you got about 6 days Before the appointment try to taper your dose . 8 mg and 16 mg are practically the same in terms of keeping withdrawals at bay the only difference is 16 mgs will antagonize other opiates stronger and it'll only give like an extra 10% effect from the bupe so you might even be fine at 8 mgs or even 4-6 so try to taper 2-4 mgs a day until you hit 8 and maybe take 2-4 mgs a day 1-2 days before the appointment as long as you take ANY amount of bupe you're blocking normal opiates so best case for you is to not take any for 36 hours at least which is hard I know

How are you able to work with that kind of pain? What do you, desk work or physical labor. I would think that you would be layed up with a diagnosis like that. I have some veteran friends who have similar issues and can barley move. I hope you find the necessary relieve you are looking for. The 60 mg morphine ER and 20 Oxy should probably do You right.

If you've only been on the sub's for a month, you should only experience slight blocking effects and those will only last for a week or two. Do a rapid taper, you can start at 8mg, and drop 2mg a day. Get down to 1mg and you shouldn't need much more of a dose than usual. You can take it up until the day before your appointment, or the morning of your appointment. If you do dose the morning of, you'll have to wait until the evening to get analgesic effects from the morphine.

Hi. I work in the Pharmaceutical Industry (Pharmacy Technician), But i work 'full-time' there basically filling the Controlled substances Schedule II- IV (Example: Adderall, Vicodin, Xanax, etc) . But after about 2 hours max, they use me for re-stocking the store... It's a pharmacy franchise, So I'm not comfortable saying. what fucks my back up, and makes me think i'm literally dying from pain slowly is when i'm doing the hard labor, lifting up 2-liter drinks, etc. For up to 8 hours sometimes. And not only my back problems. I have PTSD, GAD, ADD, Hyperventilation Syndrome, HBP, & SVT-Tachycardia. I know, pls don't judge me. idk why I was given all these illnesses.

I would never judge you, man. I know what it's like to be born with a debilitating disease too. I too lost my insurance, when I lost my job. I had an injury and was given permission to take time off work and then my boss's wife died from cancer and he had to leave for a few weeks and then HR just took advantage of the situation and put me on a "re-hire" list. They said that I didn't obtain proper permission for the time off, and conveniently my boss wasn't able to be reached, so they fucked me. Then I lost my insurance, and like you was on a lot of morphine. I was normally on 4, 100mg MScontins a day, then up to 3, 200mg after a car accident... Then my months of unemployment ran out, and then I couldn't afford to pay for my insurance. So, I was totally fucked too.
What we ended up doing, due to money constraints, is pay for the Dr.'s visit and write cheap painkillers (that work, just not quite as good as MScontin). I got Rx'd methadone for long acting and oxycodone IR for breakthrough. If find that if I take the methadone before the oxycodone IR, it doesn't work right. So, for best analgesic results, I take a couple of the 30 IR's in the morning and then a dose of methadone afterward.
The first month was shitty, like I was able to function, but always sweating. Constantly sweating. Then of course, I'd get the chills and I live where it's like 110-115 all summer long. Like 30+ days of over 100 degrees! It is like we have 5 seasons here...
But finally, after 2 months on the methadone and oxycodone IR, I'm really stable and am living my life, not laying in bed all day in pain. The methadone is DIRT cheap, price wise. And, if you're like me (and it sounds like it), you will probably have to take them for the majority, if not the rest of our lives. Which I have come to terms with lately, and understand that there could be WAY worse things...

:( Heartfelt empathy for those who suffer "unmanaged" pain through their "Pain Management" doctor. I can SO relate to your pain, your need to work, and your loss of insurance. It's hard to find anything funny after decades of pain.

I have to share this, because sometimes you gotta laugh to keep from crying.

My specialist is of foreign decent and communication is an issue. The cherry on top is that he is bat shit crazy and hates women. (yay me) He was drinking water like a fiend during my appointment yesterday. He asked me "Do narcotics make your mouth dry?" I said "No, do they, you?" He almost shit himself when he realized what he'd said...Quickly responded, "No narcotics...allergy medicines".

Well, slap my ass and call me Sally...I've been convinced he's taking his own scripts!

:eek: I was airlifted at age 17 from a totaled vehicle. I am almost 55, still suffer every breath from spine/joint issues. Add disease, multiple GI/GYN surgeries and chemo to treat said disease...I'm fucked if I surrender to pain.

I refused pain meds for decades, with the exception of acute pain from surgery, etc. for brief stints. Darvocet was the strongest I would take, but it was recalled. At age 50, I had the perfect storm. My GP since birth died. My cancer surgeon retired, then died. My colon surgeon has been diagnosed with Parkinsons @ 62. I was forced to seek PM or throw myself a Hemlock tea party.

Although I've trialed Fent, MS Contin and Oxycodone, I'm not a good candidate with my train wreck GI. MS Contin shut down my bowels. I've been hospitalized for partial obstruction with Oxy. I'm not giving up (yet). I'm now trying some CBD:THC MMJ.

My disease/trauma is not terminal. This despair IS.

Can you try multiple modalities to help strengthen your core? Swimming is a great exercise that doesn't pound your joints...excellent if you have access. Yoga stretches, even if you can't handle the full routine, are helpful as well. The "copper" back brace/belts may offer some relief, too.

Hey. And yes I'm 19 .. but I've been going through physical therapy, pain management as i said since 2008. meaning i was 11 when it started. everything progressively got worse. more diagnoses, since being jumped at school multiples times. Before i was on Vicoprofen (Hydrocodone/ibuprofen) or norco (hydrocodone/tylenol) i have liver disease so norco was a no-no. But honestly since being on MS Contin I felt so much better than being on a opioid-combination. I will agree that morphine is VERY hard on your stomach. especially if you are dosed to high at first, or if it's extended release. I have taken morphine IV through the hospital. it was didn't hurt my stomach at all. However I have a strong GI tract and bowels... I don't get any issues anymore on Morphine, i did the first month of taking it 2 years ago. and yes i do exercise for the pain. well i was. since being on suboxone this month my life has been frozen. Tuesday i hope to get my life back some... and on the Fent ... My doctor would never write that to me. i have heard horror stories with fent. I'm so sorry that you're suffered/suffering from cancer. <3

Thank you. And yes I lost my fucking private insurance when i turned 19 in december 2015. I barely paid nothing. My insurance covered ms contin, my xanax, ritalin, and all my other meds. then all of a sudden i received a letter from the affordable care act aka obamacare, that i'm being charged 695 dollars every month without being on health insurance. i was like WTF I Was on health insurance until you kicked me off. So obama care chose my health care to family planing limited. So it does nothing for me, it covers NO medications except birth control.. wtf man. and it covers abortions, condoms, blood tests every 3 years. I can't complain or talk about it anymore it's making me frustrated. It hurts me so much not knowing if i am gonna have enough money to go to the doctor. Oh and the methadone. my doctor would put me on the opioid addiction plan. Suboxone and methadone are the two drugs they write at my practice for opioid abuse. So yeah, Oxycodone, Hydromorphone, morphine, oxymorphone ALL In E.R. forms she writes no problem. but extended release meds are so expensive. I'll rank them from lowest - highest in cost price: morphine ER, oxycodone ER, Hydromorphone ER, Oxymorphone ER <- so high. Morphine ER (Ms contin) is the only reasonably priced one. Although i hope she will write 20mg-30mg oxycodone IR 90 count - 120. instant release is cheap. I know she wouldn't write instant release morphine, i wish. I have never taken hydromorphone or oxymorphone . anyways thank you for your reply.
