Ok, so apparently there are some misconceptions about Tramadol and it's metabolites. Multiple times I have read that it'd take hours for the body to to process Tramadol into it's strongest mu-opiodergic metabolite O-Desmethyl-Tramadol. Either this is wrong or Tramadol itself has stronger opiodergic effects than most of us assume.

Here is an extremely brief report of what happens when I inject Tramdol, which comes dissolved in 2ml ampules that contain 100mg each.

T + 5 sec [COME UP] I feel a rush of warmth throughout my body, my posture usually freezes
T + 15 sec [COME UP] I start feeling much more nauseous than oral Tramadol has ever made me feel.

T + 0h-12h [PLATEAU] I can nod whenever I like. Laying down with a movie will not allow me to stay awake for more than 5 minutes. 100mg more and I would be on the brink of nodding involuntarily, even when sitting upright. Every time I wake up I am overcome of waves of warmth that feel very comforting on both a somatic and emotional level. My chronic pain is not an issue during the duration of the plateau.

T + 12h-18h :\ The effects start declining, my pain comes knocking at my door again.
T I slowly come off far enough to take a 'rebound dump'

Regarding my tolerance... I take 200-500mg Tramadol per day, no other opiates. Last week I ate 16mg hydromorphone and 30mg morphine sulfate and did not feel the slightest hint at an opiate high. 7 hours later 100mg Tramadol IR and within 2h I'm flying high, nodding my ass off, having waves of warmth wake me up and put a smile on my face.

What's remarkable is that I can usually defecate daily while on Tramadol, eventhough I enjoy it's opioid effects much more than those of morphine, hydromorphone or oxycodone. I've taken all those at nodding doses, more than a few times and Tramadol blows them all out of the water, thanks to my individual enzyme situation. I don't even bother to inhibit and induce the respective CYP450s anymore since it's so damn strong on it's own. The pain killing aspect, constipating and euphoric aspects of the high all fall behind the sedating and anxiolytic effects though. I also like it for that additional antidepressant effect, the lack of a hangover (usually opiates leave me very agitated or even aggressive on the day after) and how godamn easy it is to kick with some tapering.

All this makes Tramadol a stand-out drug in my book. I feel sorry for that majority among us who find it useless due to their enzyme situation not allowing for much opioidergic activity.
