For me when tapering down Over the course of about three months I went from 4mg per day down to nothing.
My first jump was from 6mg to 4mg, that was actually easier than the 4mg down to nothing part, though I did wait a week before I began to reduce my dosages further. How I did it is I would remove .25mg-.50mg over the course of 1-2 weeks until I was at a steady .25mg-.50mg per day, after about three days of that I stopped totally, There were some withdrawal symptoms but they were so mild that aside form needing to take gravol for nausea on days 2 through 5 and some headaches and sinus issues, which a couple allergy and sinus tyloneal pills helped with it was a fairly painless process, This time.
I tried a few things to cut out dilaudid over the winter, when my mindset to stop was right but my body was so far from ready thatI failed twice, do to my arthritis I have to walk with a cane for most of the winter due to back and leg pain and stiffness so I wasn't just taking the pills for fun, which makes it no easier to cut them out.
The first time I stopped taking I was able to just stop and after a couple days of feeling crappy I recovered, the second time was way harder, the third time took a couple tries before I got it.
Mafioso has it right about talking to people, eating well and staying as active as you can, I found cycling, lots of yogurt and fresh fruit to be really helpful for me recently.
Trying things like meditation, mindfulness, deep breathing, lots of hot showers can go a long way, so can prayer in whatever form that takes for you, if that's you.
I know this thread is a few days old but I hope this might help a bit.
Good luck, if you haven't succeeded already.
Edit
I also used a few suplements starting a day or two before I cut out all dilaudid, an insomnia formula it contained, valerian and passion flower extract as well as l-thyamine I believe. Kava Kava capsules and 5-htp. I dont know if these made much of a difference but I had a pretty decent sleep wake cycle when I was taking them, they helped a bit for me, it might be worth looking into.
