Sufentanil Citrate Dosage

What would be the recommended IV dosage for someone with 3 vials of 50mcg/mL of sufentanil citrate (Sufenta)? Each vial contains 1 mL, giving a total of 150mcg in 3 mL. Since it's made for IV, this is the ROA desired. The person taking the Sufenta does about 100 to 150 mg of Oxy per day, or substitutes it for other opiates available, such as fentanyl (IV'ed often), morphine, or hydromorph.

Any help would be greatly appreciated and if there are any other threads or links concerning Sufenta dosages, please please send my way. Thanks in an advance.
 
There's an opiod conversion chart in OD somewhere. I'm kinda reluctant to advise IV sufentanil doses, don't want it on my conscience if you kill yourself.
 
When reading the side effects of sufenta(i.v.) what causes "over excitment" in the drug? (besides the dosage)is this common in all Fentanyl drugs?
