My doctor told me that for the first three days of taking suboxone, to take three 8mg tablets SL every 8 hours. and then for the next 4 days, to take two a day spaced out twelve hours apart. does this not sound like an incredibly high dosage of suboxone? I thought usually it was prescribed to be taken once a day.

It's definitely way too high a dose, you should slowly titrate your dose until you're where you need to be. It's fine to split your dosing usually a morning dose and an evening dose, it keeps a more constant level of buprenorphine in your system.

That dose is an outrageously huge starting dose. When I was started on it, I took 2 mg twice a day for the first 3 days and then 4 mg once a day after that. Two years later, my dose has been slowly titrated up to 16 mg (8 mg twice a day), and honestly, I couldn't even imagine 8 mg 3x per day and how sick it'd make you feel. At that dose, chances are you'd get some brutal headaches as Suboxone can cause them, especially during the initial phases of treatment or after dose increases, IME. Tricomb is right about twice-daily dosing though, until recently, I used to take 16 mg once a day and would actually get withdrawals within 16-18 hours of my morning dose. Since switching to 8 mg twice a day, I don't get the withdrawals and I've noticed fewer side effects.

Huge tolerance or not, that's way too much buprenorphine to be on daily dosing, maybe for induction but even for induction I disagree with those who say people should start induction at the maximum dose like 24-32mg/day, seems way too excessive and a dose titration would be much better.

A lot of doctors are fairly ignorant about suboxone, even those who prescribe it.

I was started on 24mg and he had me on it for 2 years. Just depends on the doctor. Sure after the first 2-3 days I could do with 2mg then after day 4-5 I could do literally crumbs. But when I got to a low dose (generally <8mg) I didn't get the full effect of the suboxone. I would still feel on edge like I could go into WD at any moment. But that went away after a few weeks. But something that always stuck was under 8mg would not do SHIT for my cravings. I would still crave all day. But then I would take my recommended dose and no cravings at all.

Why would you start out at 2mg and work your way up to 16mg? If anything you should start with 16mg then work your way DOWN to 2mg.

^ I was starting to get w/d's while taking the Suboxone so my doc would decide to gradually titrate the dosage up until I stopped getting w/d's halfway though the day. It didn't do much considering that I still get w/d's if I don't dose 8 mg twice a day as opposed to 16 mg once a day. Now we're in the process of working our way back down. Kinda stupid, I know, since I wanted to be off of Suboxone within 2 years.

Docs around here (D.C.) routinely prescribe that dose (24mg/day in three doses). I was on that for 6 months till I went back to the needle. It wasn't until years later that I figured out I could get by with a fraction of that dose. 4mg/day should be enough to hold you at any level of prior use, especially after the first couple of days. Just my opinion though, I'm not a doctor (of medicine).

Then again, I've seen quite a few docs over the years dispensing suboxone that didn't know as much about it as most people here.

A lot of people are idiots. With any opioid for maintenance, you always start small and work your way up. Doing it the other way is asking for trouble. Usually 2mg-8mg is enough to get rid of withdrawal for people with the largest of habits. Anymore is overkill and the belief that "more works" is just the subconscious mind trying to chase a high. You aren't supposed to feel Suboxone, you are supposed to feel normal. Suboxone is powerful shit, less is more with Suboxone.

OP, weren't you just sniffing a half gram of dope a day, max? Yea, 24mg of suboxone is way overkill for a habit like that, or any habit for that matter.

Tommy, I have a little theory (maybe others have come up with it before, I dunno): Anything over say 4mg/daily is the same dose for all intents. My thought here is that 4mg of sub pretty much covers the receptors so the remaining amount just floats around until it is metabolized by the liver. This is for everyday use; a one-time dose will be different of course. Am I on track or is that silly?

^ Most people agree that 4mg is the ceiling dose in that respect, or maybe a little higher if you are still craving, but that's rarely needed. Higher doses will likely block better, and for longer, but have more side effects, increase tolerance, and don't really do anything that a lower dose doesn't already do.
