Opioids Question concering Hydromorphone injection concentration.

Hello, Bluelighters!

I have a simple (I hope) question about Hydromorphone (Dilaudid) concentrations when injecting crushed pills.

Assuming all the due diligence is done (precise measurements, filtering, safety precautions, etc) and assuming all other factors are equal (injection vector, injection liquid, injection site, tolerance, etc) is it preferred to have a high concentration / low volume or high volume / low concentration?

Example:

1x 4mg pill crushed and diluted into 4ml liquid Vs. 1x 4mg pill crushed and diluted into say .5ml liquid?

Which is safer / better? I'm curious to hear thoughts / theories and supporting data.

This isn't quite as specific, but I have lots of dilaudid experience, not the greatest on iv safety and I only fill up the rig half way-- tiny insulin bit. For me, the rush seems better and if it fills with blood to quickly there is less issue. But dilutions seem to be more of an issue with opana, roxi, and bupe in my experience. Sorry no numbers.
 
I think a general rule of thumb in IVing is that more dilution is better than less dilution, but i don't know much on the subject.
 
Why in the hell would you ever use 4ml of water for hydromorphone?(or anything aside from morphine/methadone, for that matter)?

You realize the water solubility of hydromorphone is 333mg/ml?

In general, you should use more than the smallest amount of water possible like many use, but for hydromorohone, it doesn't take much. Using a 3ml syringe, I had no problem injecting several 4mg dillies.

With H-morphone, an advantage is how easy they are to bang. Just use however much water makes you comfortable, without using an insanely high/low amount. I personally would use 2ml of water for 4 tablets, 2.5 for 6, but I prefer a larger volume of water.
 
I knew my advice would be bad... I rarely fill them and I used to do 5 8s at once. Womp womp. Sorry.
 
Usually injection volume doesn't matter with highly water soluble drugs. It all makes it to your blood anyway. Use enough water to make a filterable, handleable solution, but don't be putting 2mg in a 500ml drip bag.

I just threw that number out as an example.

I asked the question because I'm curious and want to learn more. Personally I currently use two different dose/concentrations for chronic pain: 1.5mg/.5ml or 3mg/1ml(by volume, I make batches in vials) when it's really bad.

Just looking for insight as to what others do. Thanks for the replies so far. :)

I'd probably double the concentration if I we're you, but it depends on your dose. Really, it is just a personal preference thing. 3mg/ml is fine, but most people would use more.(less water). Just make sure it is enough to properly dissolve the tablets. And you fine.

How do you make your batches? Do you have bacteriostatic water? I don't like the idea of prepping shots in advance, unless you have all the right supplies.

I use .9% sterile saline solution. Either from the little 5ml "ampules" that are normally for inhalation(Easy to find.) or, from IV bags(Not so easy to find.). I filter with cotton first to take out all the big stuff then run the result through a micron wheel filter directly into the vial. End result is very clean and stable. I store the solution in a 10ml sterile vial. I put the vial in a sealed tube and store it in the fridge when not actually drawing a dose. I've kept a batch for as long as a week with no loss of potency or "growth" within and no ill effects. Been doing it this way for nearly a year now with no issues (knock on wood).

When I first started and my techniques were raw, I used regular distilled water and after a couple days I would see growth of some sort within the solution and had to dump it... So yeah don't do that lol.

I've spent considerable time, effort, and money investing in equipment to make batches as safely as I can without turning my home into an actual lab. :)
 
I just use tap water, whatever spoon or can I can cut up, either a cig filter or q-tip end, and heat it with a lighter if its necessary. I don't pre make anything really unless its for a couple of hours later. I know that's not the healthiest way but I feel like I'm pretty clean and I don't share anything.

I see. I get ya. I guess it all comes down to what you're comfortable with. If you're using tap water then yeah you definitely want to use it quickly because it does not have a long shelf life. I suffer from chronic pain and when it starts to flare up I generally don't have much time to get medicine in me before the pain becomes debilitating so I like to keep a batch on hand at all times.

I inject because oral BA is too low and it takes too long to have any effect.
 
Yea, I have headaches from a brain injury after about 20 min I start vomiting from pain-- I feel ya I'm so sorry. I guess I just have gotten really quick... Less than a min to make between 1-5 to shoot, sad haha.
 
Last time I bought rigs, I accidentally bought the .3ml syringes, while I typically use the 1ml ones...no problems or issues noted at all, and I do a LOT of dilly...

I usually use 1ml/100unit insulin syringes but was going to try upping the concentration and use .5ml syringes so that's why I wanted to see what worked for others. This has been very helpful, thanks everyone!
