One of the deadliest street drugs, illicit fentanyl, has transitioned from a hidden killer that people often hope to avoid to one that many drug users now seek out on its own.

The shift to intentional use of fentanyl underscores a worrying trend in the country's ongoing opioid epidemic, experts say: That a growing number of people have become so tolerant to opioids like heroin, that they're turning to the synthetic compound, which is up to 50 times stronger.

Until recently, intentional use of fentanyl was mostly limited to the West Coast, but in the past few years, addiction specialists have seen an uptick in use of the drug across the country.

"Two years ago, I would have thought this was crazy," said Dr. Akhil Anand, a psychiatrist at the Cleveland Clinic who specializes in addiction. Now, Anand said, he's seeing a lot more people in Ohio seeking out illicit fentanyl as their drug of choice.

I've had some pretty great times smoking the patches. It's not smack, bit I get the appeal.

Me and my people never got much from trying to use them transdermal. But a small amount of a 50, 75 or 100uq patch would get one high as hell smoked, IV’ed or sublingual.

Smoking fentanyl I always say is like getting hit over the head with an opioid hammer. It’s so sedating and intense. I’ve watched people go out nearly instantly and be out just sitting there but pop back up 30-60min later like nothing happened. If you nod on real dope your in and out for hours.

I feel I was one of the first few to try it honestly. At that point there was like two reports online and that was it. I took a 100uq patch, one of the rubbery/plastic matrix ones, and cut a square. Put it in my one hitter and went out back my friends house in the bitter cold to try it.

I was 16 I think at the time. My friends were inside playing video games. As I hit it, I tasted this nasty plastic synthetic taste but kept going. After a couple puffs I felt myself slowly fall back into the wall I was near and realized I was high.

Next couple hours were marked with a strong sedation and breathing depression that had me feeling like if I fell asleep I’d never wake up. Periodically throwing up in the toilet. After 3hrs it subsided.

Experiences after that were usually 1-2hrs, the first usually bringing on a nap.

The lack of euphoria (although there) was why we never really used it. I’d rather take hydrocodones than fentanyl cuz there’s actually a feel good component. Fentanyl is for people that want to sleep through life.
