I take a combination of tramadol and morphine er for chronic pain ( and recreation). I take between 90-180mg of the morphine untill my prescription runs out each month. I have done this for a long time but suddenly any good feeling that I had while taking the morphine is gone. I just feel sad. I've taken a self mandated break hoping that these feelings will go away, but they didn't. Is this typical has anyone else experienced it? Is there anyway for it to be fun again ( I don't know if this matters, but I've taken this combination for about five years)

Thats called tolerance. Theres really no getting the "magic" back from having low tolerance or no tolerance imo.

Oh your getting actual depresssion? That could be from either morphine or tram as any opioid can occasionally trigger depression
Yea & and sometimes even other things can trigger depression as well .. ie current situations, life changes, trauma … heck even some vitamin deficiencies, ect. And this all can be happening at the same time as you’re developing a tolerance.

Sorry I don’t know if anything I’m saying is helpful, just thought it as I read this so wanted to add.

I hope things get better OP, keep digging & I hope you get some answers.

Yes actual depression

I don't think it's from the tramadol I mean, I guess it could be. It seems like when I add morphine or it's the morphine alone. I get very depressed. This used to happen when I was taking morphine
and not sleeping for several days like it would build up my system and in four or five days I would feel a little depressed now unfortunately it seems like within 12 hours of taking it I'm sad and depressed. I don't even see a point in taking it although it does help pain it is suddenly just making me miserable and I don't understand why. I've checked out both therapeutical and recreationally for many years.

Thanks. That is helpful. I can't really talk about it with one in my life as these meds are kinda my secret

Morphine triggers my anxiety and depression. I have panic attacks especially on higher doses of it. This is just my experience but perhaps after being on it so long you have developed an adverse reaction to it?

@Virgogirl619, I have heard of this happening with tramadol before, have you considered dropping it, if that is possible.

Tramadol is an funny opioid, it is a pretty weak one and it also has snri properties if I remember correctly and I have seen it cause this sort of issue for other people.

Just something to think about, I hope it get's better for you.

Sounds like tolerance, most likely. How many days in a row each month are you typically using morphine and how many days are you off it?

You may also have developed a permanent response. I’ve never found opiates of recreational but after just three months of using low dose morphine (15-45mg) every three days for pain I developed hyperalgesia, and ever since taking any amount of morphine just makes my pain worse. Maybe you’re experiencing something similar, but you’re just getting dysphoria instead of the typical euphoria? Has there been any change in how effective it is for your pain or can you not tell because the tramadol is treating it adequately?

Another possibility is that tramadol was acting as an antidepressant as well as a potentiator and it’s stopped working for you. It’s not uncommon for antidepressants to stop working after one has been on them for some time, and tramadol has SNRI properties. What dose are you on and how long have you been on it for?

Chronic opioid use has significant effects on the endocrine system such as reduced levels of sex and thyroid hormones which has various psychiatric and physical implications
