How can meth seemingly create pockmarks on skin when the user does not pick or mess with their skin while high.

Is this because of meth destroying collagen and revealing past acne scarring or the effect of toxins coming out through pores??
 
Yeah even if you're not picking, it can damage collagen and elastin fibers in the skin, causing a loss of elasticity and resilience. That might worsen existing scars or acne, making them look like pockmarks.

Tons of reasons

It dries you out, and unless you're on top of hydrating that can damage the skin on its own. It also induces the body in secreting sebum oil from pores

It reduces awareness or care of ones hygiene, thus the less you shower or bathe, the more bacteria accumulates on the skin

Sleep disruption and poor nutrition from loss of appetite will take a grim toll on your organs, and the skin is no exception. Your immune system will weaken and the skin will begin to suffer

Throw all of these together, dry oily skin, excessive opening and closing of pores/sweat glands, unmoderated bacteria growth, and weakened immune system and you get: raging cystic acne and other fun skin conditions. And remember it's not enough to watch the risk factors, you need frequent long breaks from meth.
