Opioids Hydromorphone ER to oxycodone

I been taking 24mg hydromorphone (Dilaudid) extended release for back pain with 10/325 oxycodone for breakthrough pain. I am going to be two days short on the ER this month. I get my refill Wednesday 11/13. I have two more, normally that would be one day’s supply. It lasts 24 hours or more. I’ve gone up to about 30 hours with no repercussions. My question is how much Percocet will it take to compensate for the lack of ER hydromorphone? I’m aware it is much stronger than Percocet. Am I totally screwed? If I go by a roughly 3:1 ratio, does that mean I have to take 7.5 hydrocodone every approximately four hours?

Also, I had some extra Percocet and used 32 in 2 days. Would this add to my feeling withdrawal symptoms now? I thought a 2-day party was not a long enough time to get my body habituated to that level of medication.
 
Working in a Belbuca routine has helped me a ton. I was a 15 year user and tolerance went so high I actually had a heart attack eventually a heart transplant. I am still on 10/325 4 a day with Belbuca (I don't use daily but subscribed 450mc two times)

32 Percocet by itself and sudden use in one day is scary, your not on a sustainable path. I understand the extreme pain but sounds like if pain management is your end game then finding the right inputs for that are in order. Sustainable is a real thing, use only goes up until the shit hits the wall. Also I just lost my brother to methadone tolerance went up so far after 10 years his blood became septic leading to other problems and passed a young man.

The short answer to your withdraw fears, If it's something you already have tolerance to, take the "Half life" of the drug X 2-4 and that's how long it's in your system. That does not mean the harm is not done during use, that's where I got it wrong.
 
Thank you for your thoughtful reply. I agree this is not sustainable. I need to lower my dose so that it becomes treatment again instead of party all the time. I feel so much shame.
