So as I posted in another thread, it's time for my wisdom teeth to come out, as they are already causing me pain. I'm terrified of getting them removed because of the pain afterwards. The problem is, my "history" is probably well documented and I am also on naltrexone (vivitrol) therapy. But I am not trying to abuse them this time, I feel like I really will need them and will take them accordingly if prescribed. (I've done it before after a car accident. Didn't abuse the pills at all because I was actually in pain.) Is a doctor willing to prescribe opioids to a patient who has a history of opiate abuse but really has a legitimate medical need for them?

i have not had that surgery, but i recently had surgery for which the recommended amount of post-op oxycodone is higher (actually a lot of studies and institutions are recommending no oxycodone or equivalents prescribed after wisdom teeth removal), and, while the oxycodone made things more comfortable, i would have been fine without. i obviously would have been in more pain, but not a maddening nor desperate level of pain.

when younger, a lot of people would sell their entire prescription after having wisdom teeth removed. i know this because i’d be the buyer. they were not financially desperate; they thought selling pills was cool and preferred weed.

i wouldn’t worry if your surgeon decides it’s better to go without. or spend too much time worrying about it ahead of time.

Sorry, but this thread will only end in endless speculation as we don't know your doctor, plus we can't help you/tell you what to say to your doctor to get a prescription.
What I can tell you is that in germany you only get ibuprofen/paracetamol (acetaminophen) for wisdom teeth removal. As long as you cool your cheek, best right after the OP, and take some ibu/paracetamol right before you should be fine. Ime the pain is managable and you should feel back to normal in a few days.
