Opioids help rapid raper METHADONE TO SUBOXONE

I'll try to keep this as short as possible but there is a lot of stuff i need to say. So 3 days ago a presumably OD'd on methadone/ etizolam combo.( I have been on 120mg/ day methadone for almost a month now, and a month before that I started at 30mg and work up to the 120mg. Anyways on this day (3 days ago) i took a litle over my regular meth dose about 150-170mg i cant exactly remember, and a fair dose of etizolam around 6-7mg over a couple hr. period. I was at work after dosing all this, obviously I must of looked very intoxicated because someone decied to call 911 for me for a possible OD or something. Next thing I remember waking up in the hospital with tubes down my throat, catheter up the ol' pecker, the whle 9 yards.

But once i woke, or shortly after they took out the breathing tube ( thank god that was horrible), this was all the next day which was also when I was discharged from the hospital and headed back home and now here I am now. My doc cancled my MMT treatment due to me fucking up here, so I've been on a desperate hunt for a sub program, and thankfully I have found one and have set up some appointments, the first is on wednesday the 29th, 1 week from today ( I will be reviving sub that first visit.) and then the next appoint is luckily the day after that, on thursday.


Well heres the problem now, I to need to rapidly taper down my methadone, the clinic wants it to be at 30mg by induction. So I need advise on how to do the least painful taper. I think I should have enough of my last scrip of 'done to do this 1 week taper.

So what does a 1 week methadone taper from 120mg look like.? One days ago I dosed 90mgs on that day, today I have so far only taken 60mg, and am feeling fine. Can you guy help how I should proceed my taper please? i am very desperate at this point.

AS of now this is what plan I have :

W:90mg
TH: 80mg
Fr: 70mg
Sat:60mg
Sun:50mg
Mon:40mg
Tues: 30mg (AM)
Wed: mid afternoon dose BUPE ? Would this work, or am I going throw myself into PW?
 
I'm pretty sure you'd wanna get stabilized at 30mg for at least a few days to a week first and even then you'd wanna wait 72 hours after your last dose of methadone to dose your bupe due to methadone's long half life.
 
Don't do it any time under 72 hours or you could be very sorry you did. Your sub doc will understand if you've been in methadone and can't take the sub yet
 
I am no longer receiving my methadone treatment, so I dont have any options to wait any longer..I need to rapidly detox my methadone so I can swtich over to bupe..
 
I am bumping this in hope that someone please responds with some advice. Guy's, Im in a VERY tight spot right now. I need to get down my methadone dose down and QUICKLY, and let it totally drain from my body so I can start SUBOXOXE treatment on Wed the 29.

I am kicked from my Methadone, so I can no longer receive any more Methadone than what I have left to taper. I also currently live in a place where I don't have access to ay dope, pills, or any opiates on the streets believe it or not. So I am desperate for help guys.

I need help figuring out a way to taper down so that I wont enter Precipitated Withdrawals when I dose my sub.

EDIT: Today Ive dosed 50mg [Methadone] , and feel fine so far. I should add I have cannabis( concentrates ), and some etzolam to help with the taper .

Of course you're gonna feel fine on 50 mgs plus etizolam...I don't know how much methadone you have left, but I'd taper down to 5 mgs doing big drops everyday...And then(here's the hard part), wait 48-96 hours until you're in full-blown, cold sweat, can barely move withdrawal, and then start taking 2mg doses of bupe every 8-12 hours until you begin to stabilize....The more you can tolerate the pain on the front-end going into it, the better you'll be able to tolerate the bupe when you first dose it....but 50mgs with etizolam is pretty far from where you need to be to make the jump...

Normally, they suggest that you stop at thirty mgs of methadone, and then wait 48 hours, but due to the long half-life, and the experiences of many others that I've talked to, you're better off getting as low as possible, and then waiting as long as possible before attempting to induct onto the bupe...

You're net suffering to get onto bupe will probably be on par with straight heroin withdrawal...and then, you'll have to eventually taper off the bupe....

Another option is to just taper the methadone, get some clonidine and other comfort meds, and be done with it!
 
The best option is to not take any methadone at all starting now, wait until you feel like shit after a few days and are well into WDs and then take your suboxone.

The fact of the matter is methadone has a fucking insane half life in the body. It's not physically possible to get it out of your system quickly, and the fact it's a potent opioid means it's difficult if not impossible to do a "rapid methadone taper". It can take up to 3 weeks to flush it out entirely in the worst case, so there's no way you're going to be able to go from taking 30mg methadone one day to taking suboxone the next without feeling like utter shit.
 
Thank you guys for the replies. yesterday I dosed 50mg, today im taking 40mg, then sunday I take 30mg ( Last dose ) , then wait until Wednesday afternoon is my Sub appointment.

To add to this I metabolize methadone very rapidly, so that does help me somewhat here.. I wake up in WD most all morning.. so after 2 days w/o dosing I should be in pretty damn tough WDs.
 
You might have to go a few days without done unless you think you can handle pwds.

This isn't really my area of expertise, but if your in a tight spot, I know that feeling.

Personally, I would rather be sick, than fucked.
 
^^ If you were to read correctly, you would see I do plan on goin like 2.5 days without any methadone, before I dose my first bupe .
 
Well, If i can do it today is my last dose of methdone 30mg..Then I wait until wednesday afternoon ( my sub dr. appointment ) to dose my Suboxone.

I am really hoping that those days in between will be enough time to let the 'done drain my system, so i'm not thrown into PWD when I dose my suboxone....
 
what if I indulge myself one last time - take one more high im just talking 120mg, so I can feel good, ive been feeling like this this whole taper down,one last dose of methadone and that will be the last one. then I wait it out until my sub appt. wednesday>?

would this effect me for havin too much methadone in my system to get bupe?

Unfortunately, yes. 120mg of methadone is a huge does. The fact that you dose methadone chronically means it will be more like 200mg acute methadone.

It will literally take 48h to return to baseline, and another 48 hours or so to get into full dt's.

And even then, you can't dose buprenorphine, not quite yet. It'll take a few days of full withdrawal from methadone to get your tolerance low enough so that you can switch to bupe and get the full benefit.

Some will say to just wait until full withdrawals, but because of buprenorphine's ceiling effect( and being a partial agonist) it can't completely cover a significant. Really, tapering as low as possible on MD. Waiting a few days, and then transition is ideal. No, ideal is cut MD dose in half, switch to a short acting full agonist, wait a couple days, switch, but you get the point.

But honestly, I think you should take more big dose. But how much do you have left? Don't take everything you have tommorow. Take most of it, then Tuesday, take a SMALL MD dose, like 10-30. Then, wait 4-5 days.

But ask yourself if you can handle 4-5 days without before doing this, because you'll have too. A single massive dose of MD(over 100mgs) will fucking jack your tolerance up instantly, and it sets you back a bit.

If you have only 120mg left, I would suggest taking 100mg today, waiting 36-48h, take 20mg, wait at least 72h, then dose buprenorphine.

Trust me, I've had to do this shit multiple times over the years.
 
I never did that what would of been a " dumbass move" on my part, im talking about taking the large dose last night - I just went to bed instead. * thank god * lol

Anyways today I dosed 30mg Methadone, and this is my last dose of 'Done until I switch over to Suboxone.

So last Tuesday is when i had an "OD" event that landed me in the hospital ICU, but I was released the following day (Wed). I got home late that night, wasn;t able to find my methadone I had at home so I couldn't dose, and just went to bed. Next morning I find where my damn Methadone was put,Thankfully because I was starting to get into pretty bad WDs so, I dosed 90mg.

Next day(friday) I dose 70mg
Next day (saturday) I dose 40mg
Next day (sunday) I dose 30mg
Next day(monday, Yes TODAY) 30mg.

My appoint with my Sub doctor is 230pm on this wednesday. So I am pretty confident that I should be in enough WD to be able to dose my SUBOXONE, and not have precipitated WDs...

Anyways Im just looking for opinions I guess on anything else I can do to make the switch to Suboxone happen?

Just keep in mind and be prepared for a rough transition regardless of PW's. Even switching to bupe from 30mg's of methadone, many people have a rough time stabilizing on the bupe. So if it's not working right away, don't worry. Just stick with it, and in a few days it'll work better and better until you're stable on the bupe.

check it out methadone is not an opiate therefore taking bupe will nooooottttt throw u into withdrawl i jus switched from subs to meth 3 days ago nd im fine....ik people whove switched from meth to subs without a taper nd they were fine good luck
