It seems like I've been searching forever for a medicine to treat or cure depersonalization and derealization, mine is triggered from anxiety( I smoked a lot of high quality shatter plus had 40mg ir ritilan in my system, and had a nervous breakdown,)
and ever since I've tried everything from Xanax Ativan klonopin and Valium to Paxil Zoloft , and even beta blockers , but nothing seems to work, I reaserch end into medical treatment for do/dr and always see the same old anti-depressent and anti-anxiety
treatments but none seem to work I haven't tried CBT yet but I have a session in January, but I need treatment now. I feel like I'm going Down the rabbit hole of insanity I have panic attacks, I can't go anywhere because I'm too scared, do any of you know of any really approved treatments for dp/dr?? Thanks

I would say anti-psychotics or mood stabilizers or a cocktail that may include one of those and antideppressants and anti-anxiety meds.

From what I've read, antipsychotics can be effective for such disorders.

I dont think antipsychotics are of any real benefit and may actually be harmful in depersonalization disorder.

There arent any "approved" treatments I am aware of but there are some therapies that have some benefit.

For drugs, the most common approach is SSRI or clomipramine with or without a benzodiazepine (clonazepam is what Ive read mostly is used but doubt it is the only one.) I do find it odd benzos would be of benefit, but it is utilized nonetheless. Unfortunately, the data for these approaches seem to show they aren't any better than placebo for most.

Opiate antagonists like naltrexone have slightly better data but it still isn't conclusive. It is postulated the endogenous opioid system plays some role in depersonalization and hence why this seemingly unusual drug therapy may help.

Lamotrigine probably has the most studies showing positive outcomes and I imagine would be considered first line therapy. I would definitely ask about this one but make sure you read up on the risks of starting at too high a dose if you decide to give it a shot.

As far as behavioral approaches, again limited info on what works but CBT and certain relaxation techniques (sorry I forget exactly what kind but surely could find) seem to at least help a small amount of people.

Hope you find the help you need.

^^^I agree with Lamotrigine, it is in a category of meds called Sodium Channel Blockers, but I would probably try a medication called Eslicarbazepine Acetate (trade name Aptiom) instead of Lamotrigine assuming you've already tried 5HT2A antagonists like Mirtazapine.

Mindfulness meditation and regular cardio are a must IMO. The goal is to ground yourself once again with meditation, I would try it regularly for a good couple months before giving up on it. Best of luck.
