Demerol & Dilaudid questions......

BACKGROUND:
I have a very high tolerance to opiates. Been on some type of opiate ('scripts always) off and on for about 4+ years now. In the past month I have been on large doses of Demerol (100-150mgs IM or IV, a few times), Lortab 7.5mg (10+ a day some days) and Dilaudid (4mg IM one time, and 100 2mg pills since June 10 & I have another 30 sitting here to use as needed) for a herniated disc in my back. I will be having major abdominal surgery on August 14th.
QUESTIONS:
1) Is there anyway to quickly reduce my tolerance so I can achieve better pain relief with lower doses after my surgery? I will have a PCA with Demerol in it and also Dilaudid for breakthrough pain when needed.
2) I LOVE the euphoria that I get from a large, fast IV dose of Demerol as it takes the pain away so I will probably be dosing myself a lot in the first few hours after surgery. Will that affect the Dilaudids dosage requirements for breakthrough? If so how long will it be affected for?
I want to get the ost pain relief possible out of the available dosages that I will have available to me and maybe get a little joyous in there too. But I do NOT want to compromise any pain relief! Anyone have any thoughts on my situation? I'd appreciate your opinions...

For a fast blast of opiate euphoria combined with pain releif I'd opt for IV Dilaudid. That will not only take care of the pain but the euphoric rush is a beautiful thing.
If you can handle 150mg Demerol 150mg IV then a few MG of Hydromorphone shouldn't be an issue...I'm sure you can figure out your own dosage needs.
Also, why not combine? A mg or two of Dilaudid IV and a healthy dose of Demerol IV?
Just make sure you know what yer doing and don't go overboard...but you seew to have a bit of experience....let us know what ya find.
 
Thanks for your replies! Just FYI, I will be in the hospital for the first 3-5 days after surgery but my doc is so cool about letting me set my own dosage-if I need more he gives more it's as simple as that. Well anyway if anyone has anymore thoughts on tolerance, etc please let me know.

don't know about ketamine doing it but Naltrexone certainly does - opiate antagonist.
fisrt you have to be opiate free for at least 4-7 days before you take it or the naltrexone will react with remaining molecules of opiates and make you feel like you're withdrawing in the space of a few hours instead of four days or so.
naltrexone (one tablet) will then not allow you to feel ANY opiate (codiene included) for the next three days. -i tried at two days but it must be the full three, 48 hours and more.
doing any opiate will be a waste of money. but wait this is relevent.
after the naltrexone is out of your system, your opiate receptors are at a sort of newborn stage again. like you'd never used before. so the same dose may kill you or put you on the nod to near death.
this is why people keep dying on naltrexone treatment. b/c they have no clue and go and act like a drug pig by using the same dose they always did.
don't do this - take tiny amounts to test your levels.
don't know availability of Naltrexone tablets out where you are, but here, plenty of junkies have a bottle of them around that they don't use and are willing to sell b/c they're taking heroin again.
 
You're the second person I've heard mention that about Naltrexone...
Here in my part of the US, it's fairly readily available...But it's getting a LOT of bad press...negative PHYSICAL side effects, etc...have you heard anything about that in your part of the map?
