Bupe coughing up blood

Ok, you know all those wonderful health risks associated with IV'ing? Well I have a question about them.

Earlier today, I went to take my morning shot of bupe. But forgot to buy coffee when I went to the store last weekend, so I took a few excedrin instead, as I had to work and needed the caffiene. Well the caffeine apparently had constricted my blood vessels, as I had trouble finding a vein, and usually it is no problem. When I did hit a vein, it was a small one, so I went right through it, as blood shot into the syring, but I couldn't register.
Frustrated, I sat the syringe down, and ran warm water over my arms, and did some exercises to get the veins showing(as I hate to use a tourniquet) and soon enough, I had a couple of options.But by the time I picked the syringe back up a few minutes later, the blood was already separated, and starting to clot.
Now normally if blood gets in a missed shot, I will immediately attempt to hit one more time, but if I do fail the second time, I abandon the injection, and just make a new shot. But I am down to less than 1 and a half suboxone, and I can't get any till the 6th of the month, so I needed to conserve.Granted, I do have some money now to buy some full agonists if need be, but I really need to conserve my money till I get the rest of my pay, so I said "fuck it" and shot it anyway.
Well a couple of hours later, I lit up a cigarette, and all of the sudden, my chest felt like it was on fire for a few seconds, and then I coughed up phlegm with chunks of blood! I coughed up a little more, but it is clear now.
I have heard that injecting clotted blood can get into your lungs, but have also heard this won't happen with the superficial veins in your arm. So I just want to know if anyone has a clear answer if this caused this???

As always, all replies are appreciated...Well, most of them anyway.
