Hello everyone, I am sure this is one of a million posts regarding suboxones, but I am confused about something and I really hope that I can get some insight on it. Ok I have been on suboxones for over a year. I take up to 16 mgs a day. The last time I used one was Monday 5 days ago. Anyway I was having the worse toothache ever, turns out I need a root canal done and I was prescribed hydrocodone 5s. I was taking them like candy for pain but it didn't help one bit. I was thinking maybe it was because I've been on suboxones it didn't work for me. Anyway, the last time I took any hydrocodone was 19 hours ago. I am not having any withdrawals from them, maybe because I didn't feel them at all while I was on them? would it be ok to take a suboxone strip now? I'm so confused. I don't want to go into PwDs or whatever that is called. Please help....I am thinking if I took the suboxone now it would help with the pain, because nothing is. My insurance wont cover the root canal, so who knows what I will do about that. But, one thing at a time. :) Anyone deal with this before?

Hi,
I don´t think the reason you have´t felt any withdraw symptoms has to do with the fact you didn´t feel anything from the pills you´ve taken.
Once I´ve had to use Oxys for breakthrough pain, while on another medication, and did not feel a thing so at the time I took more and more. Still felt nothing.
But I did get one of these horrible withdraws right after one day. It somehow adds up in your system and the body´s response comes later.
Maybe you didn´t take so much, gladly, so - maybe that´s why you did not have withdraw. So enjoy your luck. Remember that if you keep taking it obviously your body will feel it.
Now in regards to your tolerance to suboxone, I´m not sure. It surely happens with methadone when you take just an usual dose. One builds up great tolerance. As an example, above 70-80 mg of methadone, you don´t feel anything even if you take 400 or 500 mg of oxy. But when you reach this point you will certainly feel shitty withdraws and the only way out is taking more methadone, or in your case Suboxone.

I didn't feel any pain relief upon taking the hydros for two days I took 20 mgs at once i was in so much pain. I guess I'll wait the 24 hours I'm afraid like you said the withdrawal might come later. Just the pain in my head and face from the toothache is bothering me i wanted to take the bupe for that.

Are prepared to go back to Suboxone when this is over? Wouldn´t it be a pain relief from your teeth pain?
Don´t mean to interfere, but why did you stop using Suboxone 5 days ago?

With a tolerance large enough to need 16mg of suboxone, it is understandable why 20mg of hydro wouldn't do anything. You'd probably have to be clean for months before that low of a dose would help you.

The bupe is what is messing the process up for you. Taking any full agonist on top of bupe is pretty much a waste since for the most part it will just cycle through your system without attaching to very many opi receptors at all.

Bupe has a super high binding affinity on the opi receptors & I believe only the fentanyl family had a higher one. So anything other than fent in any dosage will do nothing if your on a high enough dose of bupe to saturate your opi receptors.

Then there is the problem of using bupe as a pain medication. This is completely possible but not for someone on bupe for addiction treatment. When used for pain management bupe is dosed every 4 hrs at extremely low doses. I think it's something like .2 to .4mg every 4 hrs.

The theory behind this is that bupe will give pain relief for around 4 hrs, but bupes half life is between 24 & 72 hrs varying person to person. If you take a low dose it will not completely saturate your receptors & you will be able to get pain relief from another low dose every 4 to 6 hrs.

When on a receptor saturating dose of bupe you will get pain relief from your 1st dose of bupe but not be able to get that relief again till the bupe clears your receptors. Which will be anywhere from 24 to 72 hrs later. Since your receptors are already filled from your 1st dose any follow up doses will provide negligible results since the bupe will have a very small number of opi receptors to catch on to.

Hope this info helps some, although I know this is not what you want to hear right now.

16mgs of bupe is equal to roughly 400mgs of hydrocodone since 1mg or bupe is equal to 25-30mgs of hydrocodone according to a conversion calculator. So your tolerance is extremely high from taking such large dose of Suboxone all this time. You need to taper your Suboxone use down to a more reasonable dose such as 1 or 2mgs. Vicodin is so weak compared to Suboxone and you do have the smallest amount possible for vicodin 5mgs hardly do anything for a person not on Suboxone. Last time at the dentist i refused the vicodin and I just continued to take my Suboxone just spreading it out more. Instead of dosing 3 times a day I would dose 4-6 times a day since Suboxone is only effective on pain the first 4 hours after its taken and in small doses such as 0.2mgs every 4 hours.

I found 0.3-0.6mgs of bupe every 4 hours helped my pain immensely but you would have to spend a good while tapering to get to a dose like that if you've been taking 16mgs. Its either taper, stick with Suboxone, or stop taking the bupe for 4 months. This is just the consequence of taking such a massive dose of bupe daily 16mgs can block opiates for uo to a week after since it stays in your blood so long and even after the blocking effect is done your tolerance will be so high that other painkillers won't touch your pain and hardly help withdrawal.
