For the last 10 years l was only able to sleep with:150mg Amitriptyline, 10-20mg Zolpidem and 3mg Bromazepam. My new DR. just cut half of Zolpidem which I only used when waking up in the middle of night. Now I can't sleep at all and that affects my mental health tremendously. I suffer from:TRD, PTSD (war trauma), suicidal thoughts, chronic insomnia, chronic headache, tinnitus.... Please tell me that something new is available because I am going crazy. Live in Australia and don't leave home at all except for the DR. appointment every 4 weeks. Any suggestion is more than welcome. THANKS!

Quetiapine has been the only one that's been solid for me in the long term. Maybe mirtazapine alongside with it, but while the combo works, it does give me some seriously deranged dreams, and I don't normally even remember my dreams.

Benzos/Z-drugs work but only for short term, at least for me.

Is it true that Clozapine helps with suicidal intentions?
Yeah, it tends to numb the mind. Good for some people, not so much for others.
But if you’re feeling suicidal.. maybe being numb would be beneficial to you?

Actually, sorry, I think I’m talking about olanzapine. I took it for a couple months a couple years ago. I could barely function besides to sleep and eat… I ate A LOT.

Just saw the psychiatrist and asked him for help regarding my insomnia. The answer was:"You don't have to sleep every night." So there's no need for new suggestions.

Just got back from the appointment with my Psychiatrist. Asked him for help regarding my insomnia and the answer was:"You don't have to sleep every night". Thanks for the support and there's no need for any new suggestions.

what the fuck? can you somehow report your doctor to some board of health in your country? that is absolutely fucking insane and this guys license should be taken away immediately for even suggesting this. as someone with chronic type 2 insomnia (though not nearly as bad as you) i sympathize with your situation and wish you the best because i know how much it sucks. clonidine works very very well for me and also is apparently very useful for PTSD patients, along with ambien (zolpidem) and olanzapine if i REALLY need to be knocked the fuck out for a day. the thing with clonidine i find more so than other sleeping medication other than possibly zolpidem (because i don’t have enough in my script to take every night) is that i developed a dependence fairly quickly. it’s not a bad one, the only effect from it is being unable to sleep or really feel tired at all without taking it. since i take it every night anyway this has never caused issues for for me except when i go out somewhere and forget, but it doesn’t seem like you’ll have this issue. not sure how the healthcare system works in aus but if you don’t have insurance and there isn’t universal drug coverage then it should be super cheap. it’s also quite good for anxiety and doesn’t make me overly tired the next day, especially after you gains tolerance from a couple weeks. i hope this helps dude, and i hope you can get your psycho doctors license revoked and you find someone less sociopathic that can actually help you.

Why trazadone and remeron with magnesium work wonders for me,I don't know,I use to get so hung over off traz,Ambien is great too, don't walk out checking the mail half naked at 3 am any more

trazodone worked wonders for me and it’s not habit forming. That said, since I’ve quit drinking alcohol, I don’t need it anymore. Weed works just fine to put me to sleep. Good luck, insomnia is a mother fucker.

Unfortunately Aus. government support such a behaviour. The latest trend is to deny patients access to "toxic" medication without plan B. Doctors can only be punished for over prescribing. For under prescribing they get pat on the back.

Wow, what a swell guy, I'd wager he wouldn't hesitate to dose himself with benzos....

Sorry that didn t help, happened to me too. And ok skipping a day is not a problem like the Psychiartist said.

But patients ime in most cases describe their problem it less severe then it actually is. A dr. should check if its not reoccuring.
And a case of insomnia. Not saying you are one, mostly old people do this, so we encouraged them: describe the worst.

In 2 1/2 years my sleeping problems developed from a bit to almost 2 nights sleep a week at most. My dr. at first didn t take it serious.
It is longterm stress/ and insomnia [which are connected], and then adding Alcohol in too. Got me in seizure territory.
Which eventually happened.

My dr. once prescibed 6 Midazolam's which didn't work, way to short working anyway. After that he prescribed no mo Benzo s.
But Quetiapine/ Seroquel, well that didn t work either, but wheb i looked it up, read the side effects.
And that its contra-indicated to prescribe for sleep. Off label use that is discouraged by the dr. organisation.

So skipping a night what he/ she said vs. "Now I can't sleep at all and that affects my mental health tremendously. I suffer from:TRD, PTSD (war trauma), suicidal thoughts, chronic insomnia, chronic headache, tinnitus...." don t match up. 2-nd opinion ?

I read that your stupid doctor told you so stupid thing but if he/she one day wants to prescribe something try with clothiapine. I dont know if you have it there but is a really good pill to knock you out. Here in Spain we have 40mg tablet wich is a huge dose. With 10mg you have enough for sure. And yes its an antipsychotic medication but I didnt notice any side effect, only have no libido but I dont know if its caused by this med. Im taking this medication for allmost 5 years and you dont build tolerance. I take half tab with lorazepam and it works perfectly with my wild insomnia.

I hope your doctor give you something soon.
Good luck

And know i recall my start of sleeping again was triggered in my voluntary Alcohol detox.
Day 1 they gave a huge dose, 80 mg Diazepam. First time i slept in a long while.
Got halved next day as it was overkill, which was sequently halved, so 5 days total.

About day 8/ 9 i had my first pleasant dream in years, unknowingly of the Diazepam,
that still coursed through my body. At release, day 17 it was still detactable and felt mildly.

So I am not convinced short term use of the most suitable Benzo for re-regulating the sleep cycle,
is not possible Ime. it felt it tremensly helped.

i’m not a doctor but I think many good suggestions have been made here. Personally, I would be very cautious of any antipsychotics as these are not designed for insomnia and could have severe side-effects, including worsening of your depression.
Someone has already suggested trazodone, and I’ve heard a lot of good things about that including that it works long-term without tolerance. However, you do have to watch out for the side effect of prolonged severe erection, can’t remember the name of the condition but I believe it can damage the penis if not addressed quickly. but that side effect is very rare and usually occurs at the higher doses of trazodone used in depression treatment.
Another medication you could try is cyproheptadine, though I recommend you only leave this for emergency situations as it has a pretty long hangover, severe drowsiness and this is a very strong appetite stimulant which can cause weight gain when repeatedly used.
