Young and dumb /Couple questions - Midwest US

Just relapsed and have some health issues.. diabetes is the main health issue I have. I would like to be open and honest with my Dr about my meth intake and get some answers about how its mixing with meds ect. but don't want to get into any kind of legal trouble.. anyone know if your Dr reports to anyone when you aren't clean? Or is that covered under patient confidentially? I am a mom with custody of my kids...not sure if that falls under " child endangerment " and is a loop hole for them.
Advice needed.
I know I am dumb for relapsing..
Quitting this time is way harder then last time..
