I scare pretty easily and movies tend to that fuck with my head pretty easy too. so my list will prolly be a bit long : :)
*The Cell (creepy shit)
*The Stand (Stephen Kind, scary because of the effects and the storyline about the superflu)
*The Exorcist (still haven't seen all of it)
*What Lies Beneath ( I screamed in the theatre like 4 times :) )
*Delieverance ( I live in the south hehe)
*Natural Born Killers (fucks with my head too much can't imagine tripping and watching it!)
*The matrix, because the whole real world verus the matrix thing and how it could be true and we humans just don't know it
*Village of the Damned- those blue eyes on those little toehead kids freaked me out
well i really could go on but I guess I won't!
 
Speaking of freaked out movies - I just won a copy of Ring on video last night from my local community radio station!! I heard some disturbing news that the director of The Mexican was planning on doing an English-language remake of it... *shudders*
 
If you want to see really disturbing (fucked stuff) check european cinema in 70s and 80s... Directors like Jess Franco (really cheap), Jean Rollin (cheap but really poetic), Passolini then other italian splatter makers like Dario Argento...
really disturbing movies IMO (first that come to my mind):
Ilsa the Wicked Warden... well it's soft porn most of the time, but the final scene when proteges of the clinic for female sexual disorders eat their wicked tormentor Ilsa left me breathless
120 Days of Sodoma... Passolini's triumph of disgust... A group of nazi officers lock themselves up in a Villa on the north of Italy practicing all kinds of sexual decadence on poor youth of the neighbouring villages.
Demoni... not really scary except if watched at a cinema... Why? Because plot starts and develops in cinema.
more commercial standard films... Lemme see... Rosemarys baby (because nothing really happens, and Mia Farrow really looks neurotic) and Carpenters: In the mouth of madness (if that's how insanity looks like, I'm darn happy that I'm relatively sane)...
 
i just wanted to put another vote in for book of shadows: blair witch 2
ok the first one i saw twice, within the first few minutes i was bored, the second time i went with a bunch of people i was living with... they made me stay up with them that whole night since i was the only one not scared. i made the branch things and hung them on their doors for that one.
but i just recently caught bw2 on some hbo while nothing else was on and i was home alone bored as all hell. and it really did fuck with my head. not so much the actual movie and stupid poeple in it, just the whole basis and ideas portayed in it.
 
Sleepaway camp shocked me, thought I fugred it out then that ending a twelve year old BOY, for those who have seen it know what I mean
REquiem, I couldn't talk, kinda made me sick, espically the ass to ass scene

My parents made me watch The Elephant Man when I was about 4. The reason of course was to teach me how cruel it is to judge someone based off of a disability. However, the bag that the man wore over his head scared the CRAP out of me. His deformation wasn't scary... but that damn bag with the hole!! ::shudders:: I still get nightmares.

The Ring totally freaked me out. I've heard that they're thinking of a prequel and a sequel. This sucks cause I'll really want to see them but I'll remember how freaked out I was. I can't imagine what the original in Japanese must have been like!

It's nearly impossible to find that movie on videotape these days. I thought about watching that movie since it had an awesome cover box design. But seriously though tell us, what happened with the boy at the end???
 
I don't really wanna ruin it, I mean it is a REALLY CHEESY Horror Movie, I bought the Box set with 2 and 3 for like 15 bucks at Best Buy, 2 and 3 are the bottom of the barrel for horror movies, complete 80's all the way, the description on the box even says For Part 2 and 3 Bigger hair and Bigger Boobs, But if you want me to ruin it, you think all the way thru the first one that Angela is a little 12 year girl who is killing everyone and turns up being angela is actually a little 12 year old boy, reason you know this you see him Completely naked, his Aunt rasied him as a little girl, which if you see Aunt Martha you can tell that woman is on some really good drugs by her actions, wouldn't be that shocking except that this movie was made back in 1980, its worth atleast one watching if your into cheesy 80 camp slasher flicks, It will most surely make you laugh your ass off espically if your baked

Watched the Cell for the 2nd time whilst tripping at 2am in the morning. Best mate and I, 18 and 20, clutching each other in absolute terror about a metre from the screen in a surround sound rig.
Went to the Astor in Melbourne to catch 'Apocalypse Now Redux'. This move had me 'disturbed' for a good few hours after it. I'd seen it previously but not paid much attention, with full focus it left me quite out there...
As far as Blair Witch Goes... got handed a decentish quality VCD before anyone had said a word about it and watched it in the afternoon in a dark room on a tiny portable television through my playstation. Scariest moment of my life.
Really was truly frightening. As for seeing it in the movies with 100 other people that were only there cus it was a friday night, above the screaming kids and popcorn? Meh... I didnt even bother :p
 
I've seen almost every horror and creepy film there is, and I can honestly say these are the only movies that have ever freeked me out.

It's like watching a movie on a gameboy screen.
Even the most grotesque, mind blowing, ultra-violent, suspensful, seat clutching movie wouldn't even come close to being scary on those tiny little screens!
Watch movies on a 30" plus screen size then you'll have a higher chance of getting the full effect.
 
i was about to post "The Elephant Man" when i saw that AmorRoark did it for me. this is a great story, granted, but scared me outta my mind. had to watch it in high school for humanities.
Requiem gave me nightmares, but still, and i don't care who agrees with me, the scariest movie i have ever seen was "The Ring." it shocked the hell out of me.

I tried to watch Happiness on a whim I got from reading this very visual and performing arts forum. I couldnt watch the whole thing, and I LOVE disturbing movies. I would like to see the whole thing some day... but I honestly really don't know if I can. If anyone would care to give me a detailed summary of the last half of the movie I'd be rather happy. (I believe i saw up until a tiny bit after the father puts sleeping pills in the "gay" boys sandwich, after that I got disgusted and had to turn it off)
 
Ah, this is possibly the most attractive movie list I have ever seen (stuff i really fucking like is getting big mentions)

Happiness - Best thing to do with this? Watch it 3 or 4 times so you know exactly what's up. Then watch it with some friends who haven't seen it, hopefully squeemish too and just laugh your ass off on their reaction, it is a whole new film! :D
If you like that I really highly recommend you try to check out 'Festen'! It is not disgusting in quite the same way, but very, very black very good humour, and it really has qualities that Happiness does not have. Danish mind you :)
I liked Requiem for a dream, Fight Club, Jacobs ladder, 12 monkeys, the idiots quite a lot (that have been mentioned already)
I've been intending to watch Eraserhead for a while, getting Cannibal Holocaust too
