A while back I used codeine CWE daily for over 6 months, never had any liver problems.
From everything I've read, I don't think that acetaminophen accumalates in your body. The liver damage it can cause is the result of overdose; when there is too much acetaminophen for the liver to metabolize it into harmless substances, the CYP3A4 and CYP450 enzymes have to take over. These enzymes metabolize acetaminophen into a highly toxic metabolite, N-acetyl-p-benzo-quinone imine (or NAPQI), which causes liver death.
Chronic use of over 4 grams a day can lead to serious liver problems, as eventually your liver becomes backed up and some of the acetaminophen must be metabolized into NAPQI.
If your using codeine everyday and you CWE it, then I'd avoid alcohol as it can increase the chances of acetaminopohen toxicity. Large amounts of alcohol can increase the enzymes that will convert acetaminophen into NAPQI.
Just make sure you are doing a thorough CWE, and you should be fine. Could be useful as well to take a break every now and then, even if it's just to get your tolerance back down.
