As you know stimulants have many side effects such as dry mouth (often due to M1-4 antagonism ) and eye related issues ( intraocular pressure or pupil dialation causing sensibility to light). I think arecoline used in small doses (1-2mg) could be a good complément for those experiencing the described side-effects.

Arecoline is very nice, but not so nice on your dental health.

The effects of muscarine agonists are mostly unpleasant, especially the increased acid secretion in the stomach and the excessive salivation. The M3 and M5 receptors are an exception, with the former making your body release more growth hormone (and testosterone if you're male) and the latter increasing dopamine release in the reward centers. You could theoretically dose a non-selective muscarine agonist with selective blockers of M1, M2 and M4, but then it would be several compounds with possible unknown side effects.

Edit: actually, looks like there is a selective M3 positive allosteric modulator that probably has the same hormonal effect and also improves your glucose balance as one effect: https://www.pnas.org/content/116/37/18684

The choline supplement alpha-GPC is known to increase physical strength in addition to being a nootropic, and it affects hormone levels indirectly through the M3 receptor. Couldn't immediately find any articles about the effect of alpha-GPC on glucose tolerance in type II diabetics.
