Opioids Sudden allergy/intolerance to oxycodone and hydrocodone

I have been on oxycodone and hydrocodone for several years now taking anywhere from 15mg to 80mg a day for chronic pain. I had taken a short break of about a week and took 15 mg of oxy (legit from pharmacy). In about a half hour I felt like I was having a very bad panic attack. I was soon experiencing gigantic brain zaps. It felt like serotonin was surging from my brain. I could feel every individual hair on my head standing up. My heart would start racing every couple of minutes, I was sweating, and shaking. It felt like I had taken a bad combo of speed, MDMA and SSRIs. I felt like I was having a stroke or heart attack or both. It seemed to slow down and eventually stopped but I am pretty shaken by it.

I didnt tell my doctor because I didn't want to be taken off the opioids, so I just told him oxy was no longer working so he switched me to hydro. For about a month I was taking 7.5mg without issue, then one day I got hit with this terrible feeling and the brain zaps and heart racing returned. I took another break to give my body some time to heal and tried just half a pill and unfortunately it happened again. I am not sure what is going on here. I am not on any other medications other than doxycycline for lyme disease. I finally told my doctor and he said it sounded like serotonin syndrome but he said that shouldn't happen from taking such small doses without any antidepressants. We considered the possibility of a panic attack, but he said it wouldn't have caused the strange brain shocks.

I have had liver tests done and they are normal. I had an MRI done on my brain and it was normal. I had genetic testing done to see if I am an ultra fast metabolizer and I am not. Doctors are stumped, I am stumped. I have been pretty much shut inside since being off the opioids due to pain. Please let me know any ideas of what might be going wrong with my body metabolizing the drugs or if you know of anyone developing a sudden allergy or intolerance to opioids after years of use, please let me know.

You might have experienced some kind of paradoxical side effect (look it up). But three times in a short period of time seems to rule that out.

Have you made any changes in other medication, diet et cetera that could account for the reaction?

However, allergies to anything can pop up at all times. I don't think anyone here can dianose you, you need to have doctors involved that can physically examine you.

Thanks for the reply. I have been able to rule out diet and medications. Seen a primary care, pain management specialist and neurologist, no one knows for sure what is wrong with me.

It probably has something to do with your tolerance going down following the little break you took.

I know this thread is a couple of years old, but for anyone else who might experience anything similar, I would not be so quick to rule out the doxycycline in this situation. It was actually through researching doxycycline interactions with other drugs that I found this thread.

One of the strange side effects of this antibiotic, and there are an alarming number of them, is excessive pressure in the head, known as intracranial hypertension. I can tell you from very recent personal experience that I had something very similar to this reaction while smoking crack while I was on a regimen of doxycycline. I ended up getting a very strange surge of feelings to my head, and as described by the OP, I also felt the very real sensation of all my hairs standing straight up, an inexplicable pressure and tightness in my head and around my neck (particularly in the back of the head and neck), some intermittent heart palpitations, hand and fingers visibly shaking and other symptoms I would describe as a mild panic attack (not common for me at all). I did not feel like I was necessarily having a heart attack at any point, but the thought of stroke did cross my mind.

The episode was enough to scare me, and I laid off the pipe for a couple of days. I decided to give it another go, while still on the doxycycline. I had the same effects happen all over again almost instantly. It is worth noting that the crack itself was from the same batch prior to beginning the doxycycline, and that I had not experienced that kind of reaction before from it. Once I settled down I did a more thorough investigation into doxycycline and found that one of the less common side effects from taking it is an intense feeling of fullness and pressure in the head.

I can’t say for certain that this was all due to the doxycycline, but some of the side effects listed for this antibiotic definitely seemed to become pronounced after I had smoked the crack. I knew the feelings I was experiencing were not like feelings I had before when smoking. But they sure did mimic the side effects of doxycycline. Just a word of caution to anyone who is prescribed this antibiotic. While doxycycline is prescribed for its ability to fight a whole host of very diverse illnesses, it also brings with it more severe and numerous side effects compared to other antibiotics.
