Rapid sub taper with 12mg total subs

Hi guys . Been messing with subs mainly because I was addicted to other opiates . Subs were a way for me to save money. However, I am ready to stop relying on subs now and i am tired of being a slave to this drug. My dosage is probably around 4 mg MAX daily for about 5-6 months now. I have 1 and a half 8mg strips. What's my best way to taper and quit with the least withdrawal symptoms. It's not a huge daily habit, but we all know how potent subs are. Thanks for your replies.
