Well I'm currently taking 1000mg of Clarithromycin. A strong inhibitor of CYP450-3A4 Enzyme. I've also been taking 1mg Lorazepam a day for over 2 years. I tapered it down to 0.5mg for a few weeks but after starting the Clarithromycin, i haven't had any Lorazepam in 48 hours and I'm starting to feel fucked. Slight feelings of sinking through chairs when sat down, minor vertigo sometimes but i just feel incredibly weird and disorientated. My orthopedic specialist said today to take absolutely no Lorazepam until i speak to my doctor, because of the strong drug interaction. But my doctor wont speak to me yet so i figured i don't have many options but to ask Bluelight! Also as a side question, I have a diet high in some psychoactive herbs such as nutmeg and parsley. I don't take them recreational, but my diet is rich in them. This has never caused any problem but could this cause issues with the Clarithromycin? Just going off the basis of most drugs are around 3 times stronger with this medication. So if i was to consume 2-3g of nutmeg in a daily diet, that would be like 9g no? Or does the CYP450-3A4 enzyme not have any effect in the psychoactive compounds in these herbs?

sounds like you're experiencing lorazepam wd's man
and as to the nutmeg i'm sorry but i wouldn't know as i can't stand the stuff; imo taking any nutmeg is taking too much unless its sprinkled on my coffee or pastry ahah༼ ༎ຶ ෴ ༎ຶ༽

,5 Ativan is a relatively high dose due to potency when considering jumping from it. That being said, you might be a lucky one. Usually by 48 hours you'd have MAJOR symptoms. Your symptoms sound like depersonalization or derealization. Neither are life threatening but freaky. Monitor muscle spasms and twitching and heart rate. Try to breathe. You'll probably experience rebound anxiety. But usually 48 hours after jumping, you have an idea of how bad it's potentially gonna be. Ativan has a short half life.

Well this is where stuff gets very interesting.

I've still taken no Lorazepam approaching 70 hours or so? and today i woke up and felt pretty much fine. No withdrawals just a slight disorientated feeling like i would expect. Nothing like the symptoms i was getting yesterday.

But i never taken any Clarithromycin yesterday or today.

It appears that the symptoms are mainly from the Clarithromycin.

The foot is looking better and i was only about 2 days into the Clarithromycin, but i imagine stopping after 2 days of a 7 day course could be dangerous? considering i have a serious injury.

I'm a bit lost right now, or should i try some Lorazepam with Clarithromycin? but i don't want to feel heavily sedated!

I really need some advice guys, right now i have taken no Lorazepam and no Antibiotic because i don't know what to do. And my doctors are useless i was told today, no interaction between Lorazepam and Clarithromycin, take it as usual. But we all know that is bullshit.

Very concerned also because it says on Clarithromycin leaflet, do not take with irregular heart beat and Lorazepam withdrawal causes irregular heart beat. Really don't know what to do for the best right now.

Mental side effects was extreme yesterday though, i was even petrified to eat parsley because of a possible drug interaction. That really isn't being mentally right is it?

3 days with no benzo and doing fine tells me you're gonna be ok on that front. I'm not familiar w the other med but I probably wouldn't discontinue it without speaking w your doctor again. Is there maybe an alternative medication that can be given?

Also, do you truly need the benzo? It sounds like you do and have taken responsibly. If there is no known interaction, I'd say take it!It sounds like you're really having a lot of anxiety!!! That could be the problem.

Well I'm allergic to penicillins so i don't know what alternative they could give me. Previously when i have stopped the benzo I've experienced extreme withdrawal. Was hospitalized with a resting heart rate of 170 when i stopped 1mg cold turkey.

The problem is, i don't even trust my doctor right now. Because although they say no interaction its far from right. Clarithromycin is a strong inhibitor of the CYP450 3A4 enzyme effectively making Lorazepam levels sky rocket. How doctors can not understand this i don't know.

Its just a damn mess. They said that even if i wanted the antibiotic changing, i would have to travel 30 miles and wait in a accident and emergency department. With a leg that has multiple open fractures and swells instantly when not elevated.

If you know the two won't mix and you have to have the other, I would just wait on the benzo. You're obviously extremely anxious which could be why you take Lorazepam but I would expect extreme rebound anxiety. Your brain is bouncing back after taking it for a long time. If your symptoms are manageable, I would tough it out. Having a reaction would make matters so much worse.

You also might want to look for a different doctor when this is over. Sounds like an unhelpful moron.

Fortunately it sounds like you're getting off easy. Benzo w/d is nothing to play with. A couple years back I was using ungodly amounts of Xanax to substitute my opiate addiction. Long story short; I began throwing up bile and shortly after full-blown seizures. I wound up spending five days in the hospital with pneumonia. Benzo detox can be a very long process so if you're feeling tremors or shakes becoming more apparent or more lengthy, don't shrug it off and get some help.

Also, what I found to greatly help is Gabapentin. If you're able to come across some it could make the detox less hellacious.

I agree, this is why i am trying to hold off Lorazepam, although i haven't taken either right now!

Fell asleep for 3 hours, just woke up and felt extremely confused on waking. Someone had to tell me 5 times that its 7pm and not 7am! And i still don't really comprehend time even 20 minutes later. Time just doesn't look or feel right! Is this normal would you say?

What dosage of xanax mate? i know that high dosages can be very dangerous. I was just hoping that .5 would be possible. Although i feel mighty confused right now lol.

Gabapentin, i can get it. Will do some research.

From what i can tell clarithromycin is listed as the strongest inhibitor by the FDA. The inhibition should cause rapid levels of Lorazepam to rise after administration. I believe this increases the Lorazepam by 2.5-3 times?

What i really could do with, is someone who has knowledge on the CYP450 enzymes and has some info on exactly how much stronger it makes the benzo. So i know how much i could take of each to result in the same blood levels of Lorazepam.

get yourself some clonidine and, within reason, some gabergenics.
sorry can't comment on the clarithromycin stuff never heard of it, but if you want off the benzos clon and gabas are life savers (y)
