Codeine to bupe

Start literally with 0.5mg buprenorphine or you get sick. 1mg at most. And propably yes if you werent a fent user.
Cheers. No just codeine. Script is 4mg and I've got to take it in chemist. If I get withdrawals can I dose up with the codeine I've got left to stop them? Thanks for replying

That would make it worse. Start with 0.5mg and go up.

Fuckn ell 4mg I wish. They put me on 8mg of bupe right away for 2 days, 12mg for 2 then 16mg.

Yeah, just fuck up the receptors with maximum efficiency (fuck tolerances same time too). Someone on codeine does not need 4mg wtf. Thats like if you are doing 160mg oxy or more a day
I'm from England. Did you not want to be on a higher dose? I wanted them to start me higher. Going on to 8mg tomorrow.
Anyway, done the bupe at 5 so about 11 hours after last codeine and feel fine.

I don't think 16mg is needed for. Someone who went on a bit of a run with Dihydrocodeine at doses of 300-500mg max

OP is taking his Buprenorphine (Subutex) observed, so that limits our ability to tailor a proper protocol for him.
If I were you, I would skip the first day at the pharmacy. I understand, it's gonna suck to deal with withdrawal from the Codeine for an entire day. Be sure that precipitated withdrawal is going to be a much worse experience than waiting the 24 hours.
600mg Codeine is equivalent to 60mg Morphine in most Caucasian people. This isn't a crazy dosage or anything. It is significant though. If you make the wrong move and take the Buprenorphine too early, you're going to taste precipitated withdrawal followed by withdrawal for the rest of the day of a greater severity than had you just waited the 24 hour period.
The world of established medicine is still catching up regarding the proper application of Buprenorphine. I think it's great that they're doing it. From the perspective of a physician who doesn't understand dependence and withdrawal, I'm sure the idea of making an addict extremely sick during induction is not a big deal in the grand scheme of things. We all know that it would be significantly more difficult to apply Buprenorphine stepwise. I'm sure most folks would say "why not have one last solid reminder of how bad it can be" for good measure.
I really think this is your best course of action. You could even take a small amount of Codeine say, noon time if you'd theoretically be getting your medication in the AM the following day. You could take ~60mg Codeine just to help you with the worst of the rattles. I believe a small dose like this ~24 hours before induction would be fine.

No your probably right. I have been doing codeine, dihydrocodeine, morphine, tapentadol daily for years. Maintenance dose was 1000mg daily.
Thanks for this but did the bupe at 5pm about 11 hours after last dose and was completely fine. No precipitated withdrawals.

I actually couldn't give a fuck now as I never rally went into detail about other drugs. The dihydrocodeine run was my snapping point coz that's only one that gave me euphoria AND the rattle, coz like you mentioned inwas hitting oramorph, Morphine pills, tramadol at doses of 2000mg on the trams easy.
That's start of 3rd week I think now and haven't wanted another opiate /opioid since. My relationships are improving, good propel coming bavk into my life, family are over the fuckin moon and my mood is remarkably improved, Motivation, no anxiety, enthusiastic about life and the future so 16mh it is haha. And at the weekend I get to take home Sundays If I feel like it I snap. Up 4mh and rail it and get a very nice buzz.
